Continuity mistake: At the start of the episode, when Frank opens/closes the refrigerator door we see the door's layout, and the door bin cover is under the top shelf compartment. As he makes himself the sandwich Erin and Nicky walk in, and when Nicky opens the fridge door it's an entirely different interior style with the door bin cover at the top.
Continuity mistake: During the courtroom hostage scenes, when Raoul Delgado and Erin are on the phone with Danny there are some closeups of the gun Delgado is holding. After Delgado hangs up on Danny, when he slams the gun into Jack's temple and orders Erin to go with him, in these closeups the gun is noticeably different. (00:30:55)
Continuity mistake: After Danny shoots Delgado, he kneels down and holds Erin at her right side with his back facing the car (driver's door is open), but in the next shot Danny is kneeling at Erin's left side with his front now facing the car, as seen in the live feed Frank is watching on the monitor. Continuity mistake: When Danny gets into gunfight with Rory Brennan at Whitey's facility, keep a good eye on Rory's gun. After Danny shoots him, the gun drops out of his hand onto the ground between his legs. When Danny goes down the staircase, the gun is back in Brennan's hand, so Danny uses his foot to bring the gun to the floor...for the 2nd time. (00:29:50 - 00:30:35)

Visible crew/equipment: After Jaime goes after Malcolm, who was released again from the hospital, it cuts to Danny and Kate parked in the car across the street from the church, and when Kate opens her door the reflections of the camera and crew members are visible on the window.

Factual error: When the ambulance pulls up to the hospital after Frank is shot, it is a Chicago Fire Department ambulance, not New York. (00:02:10)
Continuity mistake: Danny drives up to the police station. Erin is waiting for him, leaning on the trunk of a police car. A different camera angle shows her standing up. Next shot she is leaning against the car again, then next shot she is standing.
Continuity mistake: Frank and Henry are up late, 3am, and, talking about how his late wife Mary never wanted to see her youngest go into the Family Business. Henry says how proud Mary was in the day of Jamie's graduation. But Mary was already dead by the pilot, when Jamie graduated.
Suggested correction: The graduation Frank and Henry are talking about is Jamie's graduation from Harvard, not from the police academy.
Character mistake: Frank implies that Joe is the oldest child, but he was born in 1977 (1x22), Erin in 1975 (6x07), and Danny 1973 (2x17). Erin also claims she wasn't born yet when the murder of Henry's friend took place in 1977 but she would have been about two years old.
Continuity mistake: Near The End of the episode Frank Reagan pulls up to the house of the woman who has been scamming New Yorkers with the fake 911 charity. Immediately before he arrives she is putting suitcases in the trunk of her car. As he pulls up the suitcases in the trunk are different.
Continuity mistake: Frank is talking to Garret in his office. A close up shot shows him sitting at his desk with his elbows on it. A wider angle shot shows him sitting up straight. Next shot shows him with his elbows on the desk again.
Other mistake: Erin is questioning a crime victim about who may have attacked her. Although she is asking about general physical characteristics to obtain a description, Erin specifically asks about the skin colour of the suspect. (00:02:13)
Factual error: When Henry is first handed the business card for Ajax Inc., it has an address of 542 W 96th St. Later in the episode, when Henry is supposedly standing outside the building Ajax is located in, you can see the reflection of Radio City Music Hall behind him, which is in the heart of Midtown Manhattan, nearly 4 miles away from the Upper West Side address on the card.
Fresh Start - S6-E15
Factual error: The NYPD Sergeant in uniform speaking to Danny only has Sergeant stripes on the left sleeve of his uniform. An NYPD Sergeant has stripes on both sleeves. (00:03:14)
Visible crew/equipment: At the hospital, after Maria tells Danny about the busboy who was arrested for the same venue and same MO, it then cuts to Jaime undercover in the store. While Jaime walks toward Marco, the dealer at the counter, the camera moves at the same pace as Jaime, and we can see the reflection of the camera on the glass (between Jaime and the camera). Continuity mistake: Danny and Jamie are at the pistol range shooting targets. The vantage point from down range shows them both shooting tight shot groups. However, when they are finished shooting, they hold up their targets; their shot groups are not as tight as the down range perspective. (00:34:01)
Continuity mistake: Danny and the ESU enter the warehouse basement to find Bobby, and when they start to return fire, the first guy they shoot knocks over the white plastic lawn chair (beside the recliner) as he goes down. Then, in the next shot facing the door, that white lawn chair is upright (not the yellow table chairs), but in the next shot it's lying on the floor again. (00:32:40)
Continuity mistake: After the accident, Baez has a scar on her left cheek. However, during the raid, the scar moves to her right cheek and back to her left cheek. (00:27:12 - 00:49:32)
Milestones - S9-E12
Continuity mistake: Jamie is arresting a subject after he drops a knife. Jamie kicks the knife away from the subject and it lands next to the brown box and under the red wording on the box. In the next frame, the knife is now just behind Jamie's right knee. (00:05:10)
Factual error: Danny shoots Frank's revolver, and as he continues to shoot, you hear the sound of a casing hitting the floor, which shouldn't happen since it's a revolver being fired. (00:11:46)
