Continuity mistake: When Steve and Danny go to Dr. Yang's office and find evidence of a struggle, they follow a blood trail out the back door. Danny starts to put gloves on as he stoops down to pick up a wallet on the ground. He only has one glove on before the camera angle switches to the view from behind him, then suddenly he's wearing gloves on both hands as he reaches for the wallet.
Continuity mistake: When the teenagers look up at Emily's window, it's half open. When they climb to it, it's completely open. When McGarrett arrives, the window is half open again. (00:05:00)
Character mistake: Owing to a common misconception, several people in the episode claim that the consulate is Russian soil. Foreign diplomatic missions are awarded special privileges, but they are not considered sovereign territory of the represented state.
Revealing mistake: As you see Danny enter his office you can see Kamekona's reflection in the glass pane in the upper right hand corner of the screen above where Danny has a couple of pictures on a table, clearly waiting for his cue. A few seconds later is when you see Kamekona burst into Danny's office to discuss something with Danny. (00:02:13)

Continuity mistake: When he picks up the NCIS agents at the airfield, Danny drives his car up behind the plane and parks very close to it. As the three go back to his car however, it is parked at least fifty meters away and parallel to the plane.
Continuity mistake: At the scattering of Malia's ashes at sea, Steve takes off his lei and in the next shot he takes it off again.
Continuity mistake: At the beginning of the episode, when Grace Tilwell gets out of the Ford Taurus, she closes the driver's side car door. But once the shots change, the door is still open and she walks around the door towards the building.
Continuity mistake: Steve is talking to a female suspect. In one shot, her hair is off her face and the light source is strong from the left. Next shot, her hair is almost covering her right eye and the light is more diffuse and not as strong.
Continuity mistake: Steve and Danny are talking to a murder suspect. She steps out of her office holding a folder in her hand. Shots alternate between her holding the folder and having her arms crossed.
Continuity mistake: Danny is taking to some Victoria's Secret models on the beach. They ask to see photos of his daughter. He is talking to them and he pulls out his cell phone from his pants pocket. Next shot, you see him reach for his pocket again and once again he pulls out his phone.
Continuity mistake: Steve and Danny are searching a suspect's house. Danny picks up a digital camera. He is going through some photos and the LCD display shows 53 photos on the memory card. The counter shows from 53/54 to 50/54. Then he shows Steve other photos and the counter shows only 4 photos. 1/4, 2/4 and 3/4.
Aloha Ke Kahi I Ke Kahi - S4-E1
Continuity mistake: Danny and Steve are talking in the car. In some shots you can see the car's door lock pin is up and in some it is down.
Continuity mistake: A Texas Ranger spreads peanut butter on a suspect in a sugar cane field. The suspect is tied up on the ground and mostly on lying his back and right side. He spreads the peanut butter on his left arm, head and chest. In a later scene, the suspect is taking a shower and you see lots of peanut butter on his back, both his arms and the back of his head.
Continuity mistake: The deceased was supposed to have a one-way ticket to Los Angeles but when they show the ticket, it is one way from LA to Honolulu.
Audio problem: After Steve and Danny are seated in Jerry's car, the sliding door they just entered through is already closed while still making a closing sound.
Continuity mistake: When Kono shoots the bad guy who then falls off the balcony and lands on the hood of a truck, his head is toward the front of the truck. Later when they bring out the female leader of the gang the guy is lying across the hood.
Audio problem: In the final scene Danny executes the man who killed his brother. Danny is armed with a snub nosed revolver. When he pulls the trigger, the sound of a shell casing bouncing on the floor is heard. A revolver does not eject empty shell casings.
Ho'oma'ike - S5-E6
Continuity mistake: Mr. Weiss is in the hospital talking with Chin, Kono and Lou with bandages covering both ears that were supposedly chopped off by Sam Cole During the conversation we see the bandages change to above his right ear showing his ear is fine. Next the shot shows bandages are covering both ears, then the shot switches again with bandages above his right ear, then back to covering both ears. (00:31:22)
Visible crew/equipment: When the gunman tries to flee in the ambulance Jerry rams the truck into the vehicle to prevent his escape, and when Captain Grover pulls the gunman out of the ambulance, we can see the boom pole/mic's reflection moving on the windshield.
Continuity mistake: After the gunmen exit the gallery hiding amongst the hostages, during the gunfire multiple bullet holes are made on the back doors of the ambulance, but all those bullet holes have vanished a few shots later,.
