Continuity mistake: When Sarah throws Cameron out of the window after the rigged safe deactivates her, she lands on the roof of a car at the bottom with her legs straight, arms straight at a diagonal angle and her head looking straight up. But four shots later, she is lying on the car with her legs bent over the roof, her arms at a horizontal angle and her head looking to the left.

Pilot - S1-E1

Continuity mistake: As the FBI agent shows his ID to Dixon, it is in his right hand, the badge end up. He holds a folder in his left hand. The camera cuts away for just a moment, then returns to show the ID in his left hand, badge to the right, and the folder is in his right hand, all without enough time to make the switches. Queen's Gambit - S1-E5

Continuity mistake: As Cameron is questioned about the girl that commited suicide, the interviewer holds a photo of one of the mostly-black posters. When we see his view of the photo, the white letters spell 'DAN', but before and after that shot, when we see his face, we can see through the photo, and it only reads 'A', which is a different poster seen earlier in the same episode. Pilot - S1-E1

Continuity mistake: As Cromartie is approaching the Dyson home, he carries an automatic rifle with his right arm. As he makes a turn toward the left, he lowers the rifle down to his side, pointing it at the ground. He hears a car start and an instant camera angle change shows the rifle is still pointed forward and upward, not down, with no time for the change. (00:32:55)

Queen's Gambit - S1-E5

Revealing mistake: Look at Summer Glau's face when she jumps off the Jeep onto the black van and punches the windshield to throw the driver out - it isn't Summer Glau. It's her stunt double, Sonja Munsterman. It isn't even Lena Headey driving the Jeep or Thomas Dekker sitting in the back.

Queen's Gambit - S1-E5

Revealing mistake: When the T-888 throws Cameron out of the back of the black van, it isn't Summer Glau. It's Sonja Munsterman, her stunt double. Her breasts are a lot bigger than Summer Glau's and if you watch the shot frame-by-frame you can see Sonja's face.

What He Beheld (2) - S1-E9

Visible crew/equipment: After Cromartie lets Agent Ellison go after killing all the SWAT officers, there is an overhead shot of the swimming pool. Next to the SWAT officer closest to/behind Cromartie, a hand emerges from the pool. The shot immediately then cuts to a close up of Cromartie.

Vick's Chip (1) - S1-E8

Continuity mistake: When Sarah, John, Cameron and Derek go looking for Barbara Chamberlain's body, they stumble upon Jessica Peck's body instead. Derek reads her ID and says, "Who the hell's Jessica Peck?" Cameron then says, "A threat to Skynet." As she says this, her hair is partially covering her right eye. But in the next shot it's tucked back behind her ear.

Gnothi Seauton - S1-E2

Revealing mistake: When Cameron and the other Terminator fall through the window, the wall that they crash into indents on impact.

What He Beheld (2) - S1-E9

Continuity mistake: In the scene where Derek shows John his past self playing baseball with Kyle Reese, Derek hits the ball towards John, John picks it up and then gives it to Kyle. As Derek hits the ball towards John, he is holding the bat at an upwards angle towards the right. But in the next shot of him, he is holding the bat at a downwards angle towards the left. The ball also travels in the opposite direction to what it did previously.

THGhost

What He Beheld (2) - S1-E9

Continuity mistake: When we see all the SWAT officers lying dead in the swimming pool, there is blood coming from one, maybe two of the SWAT officers. But after Cromartie leaves, there is a noticeable increase in the amount of blood.

THGhost

Queen's Gambit - S1-E5

Character mistake: When Dixon arrives to find Derek lying on the table with a serious gunshot wound to the chest, he immediately begins giving Derek chest compressions - without doing even the most basic assessment of Deker's condition, verifying whether he is breathing or even has a pulse. Dixon is a trained Emergency Medical Technician, not a passing samaritan. His error is against the most basic rules of his profession. (00:40:10)
