Visible crew/equipment: When Roy and Dixie are talking at the nurses' station about the "radioactive" truck driver, Johnny walks up to them, and when he removes the rope and stanchion, the actor's tape mark is visible at Johnny's feet.
Cook's Tour - S1-E4
Visible crew/equipment: At the construction site, when Roy and Johnny walk across the tower crane's main jib, there's a shot of the entire top of the crane, and we can see the cameraman at the tower peak, who is shooting the footage of the stunt guys from the overhead angle. He wasn't in the previous long shot of the crane. This does not refer to the officer who is standing on the main jib.
Revealing mistake: At World Picture Corp, when John and the guy with the broken clavicle are lowered from the overhead joists they're both wearing lifebelts buckled around their waists. The problem is that once they reach the ground, we can see the additional carabiner attached to John's lifebelt, which is also attached to extra green support protruding from under John's shirt. We can also see the victim's additional carabiner going under his flannel shirt. While Roy is on the line with Rampart, when John removes the victim's lifebelt, the extra carabiner is still protruding from under the victim's shirt and John places his hand over it. (00:27:00)
Visible crew/equipment: After Station 51 is dispatched to 'man trapped at the rock quarry', when Squad 51 heads through the tunnel-like passageway on the right, it cuts to a shot from behind Mike who's driving Engine 51, and just as the engine heads through the left passageway, we see Squad 51 on the right, and can also see the top of the camera and the other cameraman covered in a dark blanket at the back of Squad 51, as he shoots the footage from that angle.
Continuity mistake: When Johnny and Roy are at the apartment building with the broken elevator, Roy's watch is gone when they climb the steps, but he's wearing the watch when they enter the apartment, then it vanishes when they head back down. That watch reappears again, disappears and reappears until they go back up to the apartment for the second time.
Visible crew/equipment: At the start, after the boy ties a bowline knot in the rope attached to the van, when the mom gets into the van with her daughter, the reflections of two crew members, the cameraman and the top of the camera are all visible in the vent window, in two shots.
Visible crew/equipment: While rescuing the invalid, when Roy climbs out of the second floor window he suffers an electrical shock, and when he falls in the closeup we can see the crash mat that he lands on when part of it flips up from the force of his weight. Also note that all the shrubs below the window have vanished.

Other mistake: When the electrocution victim is loaded onto the ambulance, we can see there's a large piece of rectangular material adhered to the ambulance back door, with the name Mayfair printed on it. Then when the ambulance arrives at Rampart, the word Mayfair is now huge on that back door and the "M" is even over the door lock.
Continuity mistake: When Brackett asks Early to see the patient (Jamie Farr) who belongs to an occult group and has no function in his right arm, Brackett's wearing a blue/white striped tie, Early's wearing a red/blue striped tie, and Dixie's wearing a uniform dress. However, in the previous and following shots, Brackett's wearing a blue/red/gold patterned tie, Early's wearing a solid black tie, and Dixie's wearing a 2-pc nursing uniform with pants.
Continuity mistake: While Johnny, Roy, and Vic try to outrun Charlene inside Stage 3, after heading down the ladder, when Roy opens the door to find Charlene standing there, Johnny's holding the red extinguisher in his right hand and the nozzle in his left hand, but next shot they've switched hands.
Visible crew/equipment: After Station 51 is dispatched to the back hoe that flipped over trapping the driver, when Roy gets out of Squad 51 upon arrival, the reflections of a crew member and part of the camera are visible on the squad's door.
Visible crew/equipment: While Brackett is on Copter 10, after the dispatcher tells the pilot that the truck driver has been heard on the CB saying that he's trapped in the cab, repeating the same location, when we see the bird's-eye view of Copter 10 flying below, the shadow of the filming helicopter is right beside the shadow of the Copter 10 chopper, at the top of the screen. Revealing mistake: At the movie studio, when the actor who plays Dr. Ned is loaded onto the ambulance stretcher, their set's 'fake' window area is the same as Rampart's 'real' hallway later in the episode. We can see this when Brackett, Early and Dix walk down the hallway to kick Dr. Ned's film crew out of his room. Pretty funny to see the same chairs, wall, window with painted exterior backdrop and color stripes on the floor. This area of Rampart can also be seen in the previous episode 'Family Ties', when the clock radio shorts out causing a fire in the patient's room.
Visible crew/equipment: After Station 51 is dispatched to the fire at the marine salvage yard, when they arrive at the scene we can see a second cameraman on his knees beside the camera, as he is shooting footage of the ship from that specific angle, which we see during this scene.
School Days - S2-E14
Visible crew/equipment: When Chet is driving the crane at the wrecking yard, just as we hear Captain Stanley say, "Swing it in," we see Marco standing atop the cars, with the moving crane in front of Marco. The position of the camera is at the back of the pile of cars, and there is an outstretched hand at the top of the screen blocking the glare for the camera, which is tilting upwards.
Visible crew/equipment: After the guys rescue the woman who was trapped upside down in the wrecked car, when they load her into the ambulance, the location set lighting is reflected on the glass of the ambulance doors.
Continuity mistake: When Gail, Laurie, and Pete realize they forgot Melinda, Johnny is wearing his leather jacket when Roy says "Look, it's a boy," but in the next shot Johnny's jacket is gone when he asks what happened.
Visible crew/equipment: When Johnny and Roy walk into the staff lounge, to join Morton, Brice, and the other paramedics for the tape review meeting, the two actors’ T-marks are visible under the table, where he and Roy are standing.
Visible crew/equipment: When the guys wake up in the morning and walk into the kitchen, the clear reflection of the camera dolly is visible on the oven glass door as it trucks to the right, when the guys see the empty mouse trap.
Visible crew/equipment: At Rampart, Johnny and Roy step out of the elevator and walk down the hallway discussing the inheritance, and when Roy bends down at the water fountain the outlines of the lav mic transmitter, cable and even the dark strap (that holds the transmitter) around his waist are visible on the back of his shirt.

Correct, it was J. Pat O'Malley...he also played the grandfather with his grandson when their rocket exploded, and also played "Old Bill" in the episode with Ann Prentiss, where Gage saves the little girl from the burning tree house, and her mother falls in love with him.
