Continuity mistake: When Xavier and Cyclops begin arguing in the hallway, the door and doorway to Jean's hospital room are visible over and behind Xavier's right shoulder, and a chart is a few feet to the right of the door. A few shots later when Cyclops is telling Xavier that he failed to warn everyone, the chart is still visible, but now the door is missing, with the wall now appearing to be longer. (00:06:40)

Plot hole: After Professor X crashes his mini-jet onto the hospital roof and loses consciousness, he isn't discovered until Jean mentally locates him. An aircraft crashing on the roof would cause enough noise to immediately alert staff and anyone else in the hospital of his predicament. (00:09:15 - 00:15:45)

When Dark Xavier deflects Gambit's kinetically charged playing card to the ticket booth counter, a man is now inside the booth. The bottom center window also goes from being closed to being open. Plot hole: Wolverine's enhanced sense of smell should have alerted him to Erik the Red's spy on the roof. Failing that, the video feed Erik the Red is receiving from the spy shows Wolverine, Beast and Professor X looking right in the spy's direction, so they would have seen him. (00:15:55)

Continuity mistake: When Xavier is looking at Moira and Banshee from his window, as the camera zooms in on the latter two, you can see they are facing each other, and Moira has her left arm on or near Banshee's hip. When the shot changes, they are standing side by side, with Moira's arms wrapped around Banshee's left arm. (00:17:41)

Other mistake: When the text depicting the episode name appears on screen after the opening credits, it says "The Phoenix Saga Part 2, The Cry of the Banshee." It should read "The Phoenix Saga Part 3, The Cry of the Banshee." Note: this mistake may only apply to the DVD version of the episode. The episode that appears on Disney+ correctly depicts it as "Part III." (00:02:20)

Continuity mistake: When Moira is telling Banshee that Xavier has given everything for his students, she has her left hand placed on Banshee's right arm, and her right arm is down at her side. Then when she remarks that she thinks she's the only friend Xavier has, she is running her fingers through her hair. When this scene was shown in the previous episode however, she is grasping Banshee's arm with both of her hands, and she never runs her fingers through her hair. (00:02:26)

Continuity mistake: When Moira and Banshee are alerted to Xavier in distress, the building with his room is several yards away from the edge of the cliff.
