Continuity mistake: When everyone is sitting outside at the cafeteria eating lunch, in one shot Christina removes the red lid of her smoothie bottle but in the next shot it is suddenly back on. Then it keeps changing from either being on or off. (00:16:45)
Audio problem: When Alex is walking over to Derek, watch the couple (the man with the gunshot wound in his head and his wife) in the background. The wife is shouting at his husband but her lips are not even moving correctly. (00:26:20)
Continuity mistake: In the bar, when Bailey tells Derek to go sit by someone who cares, she lifts her glass up from the bar table, to her lips, twice between shots. (00:01:50)
Continuity mistake: When everyone rushes Christina out of the OR after she collapses, Izzy suddenly loses her blue scrubs hat between shots, with no time to remove it herself. (00:23:50)
Suggested correction: She did have the time to change. And you can see in the end of the scene when Bailey is talking to her in the elevator and Izzie is outside, the mask and the hat in her hand.
Continuity mistake: When Ellis Grey is shouting at George, calling him Thatch, Alex and Meredith run to the open door of her room. But in some shots you can see they are no longer in the doorway but are then suddenly back again. (00:16:55)
Continuity mistake: When Derek and Addison are examining the premature baby and then begin to argue, Addison's elbow is resting on the glass case in some shots, but is instantly down in others. (00:09:30)
Continuity mistake: When George askes Richard Webber if he could help examine Ellis Grey, the extras in the background walk down twice and suddenly appear and vanish. (00:18:10)
Enough Is Enough (No More Tears) - S2-E2
Continuity mistake: When George kisses Olivia's forehead as he 'dumps' her, he embraces her head with his hands, but in the next closeup his hands aren't on her head. (00:38:55)
Continuity mistake: When everyone sits down for lunch, Izzy's bottle of water jumps from her tray to in her hand (as she drinks) between shots. (00:17:20)
Continuity mistake: When Dr. Burke tells the interns that he's chosen George to help perform surgery, Christina's arm moves from the table to up to her temple between shots. (00:09:50)
17 Seconds - S2-E26
Revealing mistake: When Burke and Alex find out that the heart donor patient is dead, and rush to revive him, in one shot of the donor's face you clearly see him blinking his eyelids. (00:13:35)
Continuity mistake: When Alex and George are stuck in the elevator with the doors slightly open, with the gunshot wound victim, there is blue paper lining the slight opening in the elevator doors. However this paper disappears in some shots, the reappears again. (00:18:40)
Winning a Battle, Losing the War - S1-E3
Continuity mistake: When Cristina leaves the room the brain dead man's daughter grabs her mother's hand. In the next shot, the mother and daughter are hugging each other.
The Heart of the Matter - S4-E4
Character mistake: Ms. Bitzer says she's booked a trip to Iceland to experience the sun never setting. While the sun does set quite late there, Iceland is located south of the polar circle and does not have midnight sun. And either way, season four has been established as taking place after summer, so there wouldn't be midnight sun anywhere.
