Character mistake: Ms. Bitzer says she's booked a trip to Iceland to experience the sun never setting. While the sun does set quite late there, Iceland is located south of the polar circle and does not have midnight sun. And either way, season four has been established as taking place after summer, so there wouldn't be midnight sun anywhere.

Visible crew/equipment: When Meredith is walking away from Thatcher's house, there is a crew member that is between the house and mailbox who slips into the shadows. (00:33:12)
Revealing mistake: When Ray is panicking over being in the rig with his dead partner, Stan, and turns to look at Meredith, you can see Stan's eyelashes flutter as he blinks. (00:36:37)

Continuity mistake: Lexie Grey hangs sign on George's new locker, in the next shot it disappears. (00:04:37)
With a Little Help from My Friends - S18-E4
Visible crew/equipment: When Hunt and Hayes decide to get a drink at Joe's after work it then cuts to Schmitt standing at the doorway, and the blue T-mark is visible on the floor just as Kim joins Schmitt. (00:39:20)
Visible crew/equipment: When Teddy and Amelia walk over to Koracick he gives Amelia the boy's tumor diagnosis, and as Amelia walks into the patient's room the red T-mark is visible on the floor, at the bottom, left side of the screen. (00:15:10)
A Hard Pill to Swallow - S16-E11
Visible crew/equipment: After Hayes asks Meredith if she knows Dr. Yang she avoids an answer and walks away, then when it cuts to Bailey as she walks toward Koracick, three tape marks (two red, one blue) are visible on the floor near Koracick. (00:36:30)
Visible crew/equipment: After Derek tells Cristina that her patient is paralyzed, when it cuts to the ICU as Cristina talks to the man's wife, the pink T-mark becomes visible on the floor at the bottom of the screen, near the right side. (00:02:05)
Visible crew/equipment: At the start, when Jackson and Maggie react to the fire alarm, just as Maggie says, "All right fine. It's the guy. The bad, escaped rapist guy," the white T-mark is visible on the floor, before she stands up. (00:00:35)
Could We Start Again, Please? - S11-E7
Visible crew/equipment: In the ER, when they move Hattie off Norris he grabs hold of her hand, and two shots later in the closeup as Norris lets go of Hattie's hand, the white T-mark is visible on the floor. (00:04:55)
Hook, Line and Sinner - S6-E20
Visible crew/equipment: Lights, a camera, etc. are visible in the reflection of the helicopter medic's helmet. (00:08:45)
Continuity mistake: Addison removes her glasses twice from one shot to the next, when she is talking to Preston Burke.
The Other Side of This Life (Part 1) - S3-E22
Visible crew/equipment: When the two characters are talking in the car going down the road, you can see the reflection of some crew members in the sunglasses. (00:13:00)
Continuity mistake: When Hunt and Warren are in the elevator, Warren is wearing normal clothes. When the storm into the O.R. Warren is suddenly wearing scrubs. There is no way he could have changed into scrubs fast enough to get to the room before they started cutting the bars off the guy on the table.
Continuity mistake: When Addison first comes to Seattle, she mentions that she likes Derek's hair. She says it's longer than usual, looks like Russell Crowe. In the flashback of the night he left his hair long like it always is. Actually it seems a little longer in the past than current.
Continuity mistake: When the episode ends and Jo and Alex are throwing balls at the wall, the position of the bag below changes between shots.
Revealing mistake: When Alex is talking to Rebecca Pope at the end of the episode, we hear Alex say "Now I know more about you than my friends." Watch Rebecca's left eye on the right if the screen when she says, "That's pathetic." Someone did a bad job of editing the CGI and we see a phantom eye over her left eyebrow.
Other mistake: There are two mistakes when Grey is at the computer trying to figure out why her patient is having seizures. The camera pans out showing Bailey walking up - if you look at the computer it shows a login screen, but when Grey shows Bailey the screen it changes. Next one is when Grey blows up the pic of the taratoma - when they leave the picture on the computer it's actually a brain.
The Self Destruct Button - S1-E7
Continuity mistake: While Meredith and Izzie are in the kitchen discussing Derek, Izzie sets the freshly frosted whole chocolate cake on the counter, but when she takes a seat in the closeup there's a slice of cake already on the plate, and in following closeups the cake is either whole again or back with the cut slice. (00:39:45)
