Continuity mistake: When Libby kisses Hurley at the cliff, there is a strand of Libby's hair which you can see hanging freely over her left cheek when the camera is on Hurley. When the camera is on Libby you see the same strand pinched between their heads. Then as the camera switches back on Hurley the strand hangs freely again. It's not possible that this happened only through the force of wind as their heads are stuck together quite tightly throughout the whole kiss.
Continuity mistake: In the first minute of the episode when Sayid finds the wire on the beach, his left pant leg is not torn. When he follows the wire into the jungle, just before he trips Danielle's trap, the left leg of his pants is already torn, in exactly the place where the branch will stab him. (00:02:50)
Factual error: This is across the whole show, but made perfectly clear in the episode "The Whole Truth" (Season 2). Korean women do not change their last names upon marriage. A woman in Korea cannot be identified by name as someone's wife. And they would never be addressed that way. But Sun's fertility doctor addresses her as "Mrs. Kwon". Korean women can adopt their husband's family name if they move to a country where that is commonplace, but Sun was still in Korea at this point, and even after they crashed on the island, she had only just left the country.
House of the Rising Sun - S1-E6
Revealing mistake: When Charlie is stood on the bee hive, all the bees are clearly CGI. (00:10:00)
Continuity mistake: In the scene where Eko is walking up to the church and the woman tells him if he buys a statue the money will pay for a polio vaccine for the village, he is wearing an earring in his right ear. Once inside the church talking to his brother, his is wearing the earring in his left ear.
Factual error: The neurosurgeon introduces himself as "Doctor" Woodruff and also has this written on his coat. In Australia, where this scene is set, surgeons are always addressed and referred to as "Mister" after the British fashion.
The End (1) - S6-E17
Audio problem: When Desmond, Locke and Jack are standing outside the cave with light, Desmond says "maybe I can find a way out for you too," or something like that. Jack replies, "there are no do-overs." The camera then goes to over Jack's right shoulder as he says, "I tried that once." However, the words don't match what his mouth is saying.
Continuity mistake: When Miles is sent to deliver "The Package" (i.e. the dead body) to Dr. Chang, after the body is taken out of the van, Miles is seen closing the door to the back of the van. After a return from a flash forward and back to 1977, the door to the back of the van is open again.
Continuity mistake: Season 6, episode 8 "The Package": When Charles Widmore hands Jin the camera and tells him to turn it on, the lens is already out, therefore the camera is already on. When the shot cuts away you can hear him turning the camera on and the motor extending the lens.
Live Together, Die Alone, Part 2 - S2-E24
Continuity mistake: Penny's letter to Desmond changes as he reads it. The text stays the same, but the handwriting and position of the words on the page change between the two shots where you can see the letter from the front. Also, for the audience the letter is read aloud with Penny's voice. Interesting is the fact that there is one sentence omitted: "Sorry to be so dramatic, but these are dramatic times, are they not?" (You can read this sentence in both "versions" of the letter though).
Live Together, Die Alone, Part 2 - S2-E24
Continuity mistake: As they are walking through the jungle, Sawyer goes to pick up a doll. He reaches down and nearly touches the doll. But in the next shot Kate grabs his arm, to stop him, before he even bends over. (00:18:00)

Continuity mistake: In the jungle while Sayid is tied up, when they are told to leave, Jin's handcuff is higher on his wrist. As he walks past Sayid the handcuff is so low around his hand that it would slip off easily. (00:26:45)
Continuity mistake: When we first see young Jacob holding the white stone with his brother, the stone he has changes shape from long and rectangular to square.
Ab Aeterno - S6-E9
Continuity mistake: Season 6, episode 9 "Ab Aeterno": When Jacob pulls Richard upright after dunking him, his hair flies back over his head, leaving his face fully visible. The shot changes to a front view and Richard's hair is now covering most of his face.
Continuity mistake: When Sawyer dumps out the water canteen Juliet tosses him, his hand moves from the top of the base to the bottom, and the angle at which he pours the contents out changes, as well. (00:24:15)
Chosen answer: None of the main characters died in the plane crash. Many died on the island after the crash (Jack, Charlie, Sun, Jin, Daniel, Juliet, others), some lived on the island for an apparently long time after the crash (Hurley, Ben, Bernard, Rose) but some lived lives off the island after the crash (Sawyer, Kate, Claire, Miles, Aplert, and Lapidus). Remember that Christian tells Jack that "time has no meaning here," (in Purgatory). When everyone meets at the church, they are at the end of their lives however long that may have been and will now "move on" together. They look like they did on the island because that is the way they best remember each other.
