Continuity mistake: When Rand brings Sulu the plate of food, it has a bowl of colored cubes on it. Then there's a cut to a close-up of Sulu eating the cubes when Green is staring at it, and the bowl is now a plate. Then in the long shot it's a bowl again. (00:24:22)
Revealing mistake: In the jail cell scene, when Kirk tears a strap from the bed, his left handcuff is open. It remains open for several seconds, while he tries to conceal it by moving his right hand to block the camera's view.
Continuity mistake: When Kirk is facing the Horta he has his phaser in his right hand when seen from the front but when seen from behind he has his communicator in his left hand and no phaser. (00:31:00 - 00:38:00)
Visible crew/equipment: When Kirk is thrown into jail with Spock you can see the movement of the entire blue crash mat used to break his fall.
Mudd's Women - S1-E7
Continuity mistake: During Mudd's hearing Eve's sitting in the chair beside Mudd, but when Kirk states that the hearing is closed and that Mud will be handed over to legal authorities, in the wideshot Eve's chair is empty, but in the next closeup Eve is still sitting in the chair. (00:18:15)
Where No Man Has Gone Before - S1-E4
Visible crew/equipment: When Mitchell tells Kelso that Dehner is a "walking freezer unit" we can see wires and other equipment on the floor, in front of the helm and navigation console. (00:05:35)

Continuity mistake: While Kirk's trying to convince Ruk that Korby's creating the same danger just like the Old Ones, when Kirk tells him that humans are full of unpredictable emotions, the closeup of Kirk is a flipped shot (the jumpsuit's blue and green switch sides).
Operation -- Annihilate! - S1-E30
Visible crew/equipment: When Spock and McCoy walk into Kirk's quarters, in the wideshot the white tape mark can be seen on the floor under the desk at Spock's feet, before Spock requests permission to beam down to the planet, and can be seen again when Kirk stands up.
The City on the Edge of Forever - S1-E29
Continuity mistake: When McCoy arrives in NYC while the man is stealing a bottle of milk, in the closeup the actor's ring finger is partially amputated, but when he steals McCoy's phaser his finger is intact in the closeup.
The Conscience of the King - S1-E14
Continuity mistake: After Kirk introduces himself to Lenore at Leighton's cocktail party, when she tells him, "Karidian is a most unusual man... That was my father," Lenore holds the goblet in her left hand as she takes a sip, but next shot the goblet is suddenly in her right hand as she sips.
Visible crew/equipment: After Spock asks Scotty if there's enough battery power to electrify the exterior of the ship, Spock tells everyone, "Get to the center of the ship. Don't touch the plates. Be sure you're insulated," and just as Spock turns in the rear compartment we can see the edge of the shuttlecraft set behind Spock.
Where No Man Has Gone Before - S1-E4
Revealing mistake: After the ship enters the galactic barrier, when Kirk orders "navigation on automatic" there are flashes of light, and in a closeup of the first console just to the left of the main viewscreen, we can see the special effects material spread across the panel below the controls, for the fire to burn when it explodes. Then it cuts to the wideshot as a second console (near Spock's station) blows up, but in the background that first console is intact - no fire/damage whatsoever (it explodes again in a following wideshot). The Trouble With Tribbles - S2-E15
Revealing mistake: When Kirk finds his position on the bridge occupied by the Tribbles, he calls for Dr. McCoy, and as he arrives, Kirk thrusts some of the Tribbles under his nose. In the shots where Kirk's face is seen, one of the Tribbles he is holding features a visible seam.
Let That Be Your Last Battlefield - S3-E15
Revealing mistake: Stock footage strikes again when Kirk orders the red alert. A shot of the bridge and view screen, with Kirk standing in front of his chair, once more switches Kirk out for a stand in, and Chekhov has gone and is replaced by what looks like from behind like a young woman or a man with a long neck. Also the central display between navigator and helm is now different from that earlier. (00:11:15)
Visible crew/equipment: When McCoy gives the guard a shot and he and Scotty break into Engineering, you can see a crew member's hand holding a red handle and their bare arm (it's not Scotty), who apparently is helping to open the door. (00:23:00)
Continuity mistake: When Kirk's arrow hits Kras it protrudes from his leg, but in the next wideshot as Kras screams and stumbles the arrow is quite gone from his leg, but three shots later the arrow is back in his leg while he tries to pull it out. (00:43:40)
Visible crew/equipment: At the hangar deck, after Kirk introduces Sarek to Spock and McCoy they all step forward, then as Sarek introduces his aides and his wife, the three white tape marks are visible on the floor behind them, where they had been standing. (00:02:15)
Visible crew/equipment: After Amanda tells McCoy about Spock's pet sehlat, when Sarek and Amanda walk away, the actors' multiple white tape marks are visible on the floor, under the table. (00:10:50)
This Side of Paradise - S1-E25
Continuity mistake: When Kirk and Spock successfully activate a subsonic transmitter, it cuts to a closeup of a spade's metal blade breaking the soil ground, but in the next wide shot Sulu and DeSalle are using two spades made entirely of wood. (00:44:45)
Chosen answer: If they're in orbit, they're being pulled along by the planet's gravity well, therefore, impulse engines would only be used for minor corrections and would be "on standby" while in orbit, but not active. (Like keeping your car idling without revving the engine and creating plumes of exhaust).
