Visible crew/equipment: After Cloud Dancing leaves to search for the root and Dr. Mike gets the medication for Sully's pain, when it cuts to Jake and Birch talking inside the barbershop, the reflection of filming equipment is visible on the windows. (00:16:50)
The Great American Medicine Show - S1-E8
Visible crew/equipment: While Myra is lying on the table before her surgery Dr. Mike tells her she's almost ready to begin, then just as Horace is about to stand up the orange T-mark is visible on the floor. (00:38:30)
The Healing - S1-E5
Visible crew/equipment: When Loren is at the clinic talking to Dr. Mike about the "little pain" that started from lifting the piano, the shadow of the moving boom mic is visible on the wall and door. (00:09:45)
The Healing - S1-E5
Continuity mistake: Before Loren's operation, when Dr. Mike shows Jake how to administer the chloroform, she holds the bottle in her left hand and the rag in her right hand, but in the next shot both items have switched to the opposite hands. (00:21:20)
Visible crew/equipment: When Kid Cole removes the noose from Jon's neck and helps him off the horse, in the next overhead shot there are multiple T-marks visible on the ground before the shot fades out. (00:37:50)
Continuity mistake: When Dorothy is doing laundry, she grabs a large piece of linen from the washtub, as Dr. Mike approaches and they talk about Dorothy hiring a caregiver for Loren, during his stroke. As Dorothy starts cranking the linen through the wringer, she stops to protest to Dr Mike about it but then turns around holding the wrung linen in her hand, though she hadn't finished cranking it out. (00:32:30 - 00:33:20)
Visible crew/equipment: When Sully visits Mike at the clinic, after dark, to console her over Dorothy's health condition. Watch as he moves past the window. You can see two control room blue screens in its reflection, alongside a boom mic, and the shadow of a long-haired crewman, who stands up, and then sits back down. Though the camera moves behind a potted plant on Mike's desk to conceal it all.
Continuity mistake: Belle's horses change in the end. At the hideout she leaves on Thunder and later is back on the brown horse she started with.
The Tempest - S5-E10
Other mistake: After getting the horses inside the barn, Mike sees the twister and hurries inside the homestead to grab Katie and take cover under the staircase. When Sully and Brian arrive the next morning to find debris littering the house, they call out for Mike, and she comes rushing down the stairs holding Katie, instead of coming out from the staircase where they were safe.
Suggested correction: By morning, the tornado has already hit and its threat is long passed, so there was absolutely no reason for Michaela and Katie to have remained hiding under the staircase. Prior to that scene, Sully and Brian were on their way back home, and when they took refuge from the twister, they released their four horses to instinctively find their own safe place. After the tornado passed, once it was safe to ride again, Brian and Sully needed to first find all their horses, and then continued riding back home. As you said yourself, "Sully and Brian arrive the next morning."
Fifi's First Christmas - S4-E12
Continuity mistake: Brian's hair is the standard bowl cut he's sported throughout the series to this moment, even on meeting new girl Sarah and their piano lessons. When Sully, Mike, and Robert E find him mounting a horse to go look for lost Sarah, it's suddenly gelled back for a more 'older' look.
The Comfort of Friends - S6-E4
Continuity mistake: When Michaela enters the homestead after handing Katie off to Matthew, it immediately cuts to her in the master bedroom upstairs, pouring water into the bowl to splash on her face, as she suffers another pregnancy attack when she wouldn't have been able to manage climbing the stairs in her delicate condition.
Audio problem: The audio goes weird when Loren stands up to show the townsfolk the Travelin' All Stars sign during the meeting, from half-muted to full to half-muted again, like he's obviously wearing a body mic in the wrong spot instead of being under a boom.
Where the Heart Is (1) - S2-E6
Factual error: During the arrival at Beacon Hill, Colleen tells the girls during tea that the streets in Colorado Springs are all dirt, not the cobblestones as corrected by one of the girls (when Colleen referred to the Boston streets as rocks). When, in fact, several road scenes around Boston in several scenes are dirt or dried mud.
Factual error: Horace catches the ball during the first half of the final competition as Otis approaches 4th base/pitcher's mound, which outs him automatically yet Michaela nor the team call it.
Suggested correction: He was out. It was obvious that he was out when Quinn said he caught it. There would be no point in having to call him out. Otis is then seen telling his players to return to their bases because they could be tagged out. But he doesn't bother going anywhere because even he knew he was out. And we do see the players get called out.
For Better or Worse (2) - S3-E29
Other mistake: Cloud Dancing gives Sully his white Cheyenne tribal dress to wear as a wedding suit, but Sully pulls on a very different deerskin coat inside the barbershop.
Wave Goodbye - S6-E5
Plot hole: Dr. Mike and Daniel are discussing whether Princess Nizamoff is a fake or not, considering all the voices from the dead she performed during the séance. Yet Mike totally forgets that Mizamoff addressed her former medical professor in grave detail. Something the Princess would not have known otherwise.
Factual error: Near the end, when Horace is delivering Lincoln's Declaration speech, Mike and Sully are talking, you can see their breath. Indicating this was shot during the winter, when it's supposed to be July 4th Independence Day.
Plot hole: Worst writing of any episode. Mike is more than familiar (and associated) with public law and certain rights, given her position as a licensed physician, and the factor that she herself, and members of her extended family are lawyers or in knowledge. Yet Ethan is able to trip her up repeatedly during the course of this trial, incl. 'lack of proofs' etc. Whence Mike should have been able to rest her case without worry or thwart.
