Continuity mistake: During the scene where Carol and Mark are arguing with Doug about him examining the girl, the door behind her is closed. At the end of the scene, as Doug walks away, the door is open, even though no one went near it.

Ghosts - S3-E5

Continuity mistake: Abbie is asking Peter and Carter questions about infant developmental stages and they are standing up straight next to the crib. When she says "don't worry, Dr. Benton", the shot shows Peter with one elbow draped over the crib and Carter is leaning over with arms folded. They could not have changed so quickly in real time.

Fear of Flying - S3-E6

Continuity mistake: Mark is watching Susan get on the helicopter and he hooks his Flight Physician jacket over his finger so it is hanging over his shoulder completely down his back. However, the shot from behind shows the jacket partially draped over his left shoulder - then it goes back to being down his back.

No Brain, No Gain - S3-E7

Continuity mistake: Carter is working on a patient who has swallowed a lot of taffy - he is sitting behind his head with both hands working on his mouth. But when Anspaugh walks in he suddenly has one elbow up in the air and the other hand wrapped around the patient's chin. There was no time for the change in position.

Homeless for the Holidays - S3-E10

Continuity mistake: Nick the dog is standing in the sink and Mark and Doug have got him just about completely covered in soap. The dog jumps out and runs out of the room - by the time he reaches the hall, only a couple of seconds later, the soap has almost completely disappeared.

Night Shift - S3-E11

Continuity mistake: When Carter and Doyle are running down a corridor to the patient having a heart attack, the camera follows them along the floor and you can see several coloured lines going down the hall. After we see this, they burst through a double door, and if you look behind them you cannot see these lines anymore; they've simply vanished.

David Mercier

Post Mortem - S3-E12

Continuity mistake: When he's scuffling with the homeless man in the trauma room, Carter's stethoscope slips off his shoulders and drops to the floor. By the time Mark finds him standing outside, it's back around his neck, with no moment when he could have picked it up.

Fortune's Fools - S3-E13

Continuity mistake: Benton is talking to Carla about her preganancy test and they go through the double doors to the snowy outdoors. Carla has nothing in her hand when she pushes the doors open, but once outside she is holding a hat - she wouldn't have even had time to pull it out of her pocket.

Fortune's Fools - S3-E13

Continuity mistake: Dr. Hicks is standing in front of the surgery white board talking to Carter and she says "Care to join me?" He replies "Love to" and the shot goes right back to her - she has moved 8 or 10 inches to the left, without enough time to have done this in real life.

Fortune's Fools - S3-E13

The Long Way Around - S3-E15

Continuity mistake: Carol is working on James' injury and Duncan goes to answer the phone. James has his arm wrapped around the phone but the shot changes to focus on Robert - we can still see James but his arm is now at his waist, even though he had no time to change positions.

The Long Way Around - S3-E15

Continuity mistake: While Carol is trying to save Mr. Novotny's life, Mrs. Novotny's hair alternates from being loose and hanging over her cheeks to being neatly tucked back into her bun. In fact, a few minutes later when Carol asks for her keys, her hair looks just-brushed shiny and newly coiffed - it's unlikely she would have fussed over her hair at this stressful time.

Visible crew/equipment: After Dr. Benton talks to the police detective and then goes to see the grocery store owner, as Dr. Benton swings open the doors, the camera crew can be seen in the reflection of the glass of the door.

Answer: A man named Paul Sobriki comes into the ER in the season 6 episode 13 " All in the Family." He is complaining of headaches, Carter and Lucy decide to rule out meningitis and do a lumber puncture. Paul struggles during the procedure and believes that both doctors are stabbing him. Lucy, who is interested in Psych thinks something is wrong with their patient mentally, but Carter is dismissive of her. She gets cross with his attitude and she calls the psych department anyway to have a look at him. Later on and near the closing 20 minutes or so of the episode, all the staff start playing loud music and get a cake which appears to be blue lol, together to party as it's Valentines Day, Carter returns from somewhere and asks where Lucy is. He goes to check one of the rooms for her and Paul Sobriki comes up behind him in the dark and stabs him twice in the lower back, he collapses and as he falls unconscious he sees Lucy on the ground also. In the next episode, Be Still My Heart (ep 14), Dr Weaver discovers them and all of the staff work to try and save them, whilst Carter survives.
