Earthshock - S19-E6
Continuity mistake: In episode four, as they approach the TARDIS, the female member of the remaining troopers is grabbed and doesn't make it inside. But when the troopers do get inside the TARDIS, it's a man that's missing.

Earthshock - S19-E6
Factual error: The Cybermen sets four explosive charges around the outside of the door, yet only the middle part of the door is damaged...
Earthshock - S19-E6
Continuity mistake: The mouth panel of the Cybermen costumes was originally intended to be a clear perspex panel. During filming, it was felt that this showed too much of the actor inside the Cyberman costumes. So a decision was taking - mid way though filming - to paint over the 'clear' panel with silver paint. However, this does result in the mouth panels of every Cyberman alternating from 'clear' to 'silver' from shot to shot.
Earthshock - S19-E6
Revealing mistake: During the "cliffhanger ending" at the end of episode 3, it's blatantly obvious that the three columns of advancing Cybermen are merely mirror images of each other. The head of each column has black head bars indicating leadership, and is holding his gun in exactly the same position.
Earthshock - S19-E6
Revealing mistake: When Lieutenant Scott kills the second Cyberman on the bridge, he starts falling down way too early.
Earthshock - S19-E6
Revealing mistake: The "dead" crewmembers at the end of episode two can be seen breathing.
Earthshock - S19-E6
Visible crew/equipment: Just after the Doctor has explained the anti-matter to Adric, the shadow of the boom mike is visible in his blonde hair.
Time-Flight - S19-E7
Factual error: The Pleistocene Period began around 1.8 million years ago, not 140 million years ago as stated.
Plot hole: In "The Five Doctors", Susan tells Turlough that she can pilot the TARDIS to some extent. During series 1 and 2, it was clear that she couldn't pilot it at all. She couldn't have been taught between episodes as the Doctor had little idea himself at the time.

Revealing mistake: When a Cyberman's arm is sliced off by the Robot's shuriken, you can see his real arm bulging out at the right side of his body. (01:00:25)

Other mistake: The Cyber Leader is supposed to follow the Master's footsteps exactly in order to get across the chessboard. He doesn't. This cannot be classed as a character mistake because the Cyber Leader knew what was involved. I believe this mistake was caused by the actor not being able to see the chessboard through his helmet.

Continuity mistake: After the Raston Warrior Robot has killed all the Cybermen, watch the dead Cyberman in the back right of the picture. His arm position changes as the robot teleports away.
Revealing mistake: When the crack in the wall breaks open, the wall wobbles rather obviously.
Visible crew/equipment: As the Doctor is watching Will emerge from the priest-hole, look over his shoulder at the far corner. Someone or something appears to be moving in the shadows there.
Other mistake: What happens to Tegan's bag when the plague victim transforms into the cavalier? It doesn't appear to be left behind, even when the cavalier later vanishes.
Resurrection of the Daleks - S21-E4
Audio problem: Davros' lips don't move when he says, "Now for The Daleks," just before he releases the Dalek-killing virus.
Resurrection of the Daleks - S21-E4
Revealing mistake: When Lytton and his men approach the self-destruct chamber in episode three, the door is closed in front of them. It wobbles as it closes.
Resurrection of the Daleks - S21-E4
Revealing mistake: When the Doctor is shooting the Dalek creature hidden under the coat, he is obviously using blanks because when you see the coat there are no bullet holes in it.
Resurrection of the Daleks - S21-E4
Other mistake: When the Doctor's mind is scanned, we see his previous incarnations and companions, but Leela is not shown. Katarina is represented by a black and white photograph.

The Caves of Androzani - S21-E6
Revealing mistake: When Stotz cuts a hole in his ship's door, you can see that the inside of it is made of white polystyrene. They painted the outside of it but obviously forgot about the inside. Also listen to the sound it makes when Stotz punches the hole through - it sounds exactly like polystyrene.

Answer: In 'The Five Doctors', three separate Cyberleaders are definitely used. So it's likely that Cyberleaders are like unit commanders, of which a fair-sized army might have several.
