Visible crew/equipment: At the scene of the plane that hit the building, after the explosion, when Foam 10 arrives there's a closeup of the firemen using the foam tubing, and behind the doorway one of the 'firemen' kicks the foam tubing, but notice that he's not really a fireman, he's actually a crew member who's wearing a turnout coat, though he's also wearing jeans and work boots.

Visible crew/equipment: While en route to the woman trapped on a wire, the reflection of the cameraman is visible on the windshield after Mike drives past the entrance to Commercial Studios. We see it's not Captain Stanley, but instead it's someone wearing a white shirt, holding up something (presumably the camera) with both hands.

Visible crew/equipment: At World Picture Corp, after the rescue of the guy with the broken clavicle trapped up on the joists, Morton tells Roy to transport the patient as soon as possible. It cuts to a shot of Squad 51 following the ambulance to ER receiving, and as they drive through the underpass, it's from the POV of inside the squad. Some location lighting becomes visible on the right side, and the reflection of the cameraman's legs are visible on the windshield, as he's seated in the passenger seat. We know that one of the paramedics is in the ambulance, so there should only be a driver in the squad.
