Audio problem: The stick joust is supposed to occur in the middle of the Spanish countryside far from civilization - yet we can hear motors running in the background. (00:41:00)

Men Into Space (1959)
Earthbound - S1-E17
Audio problem: McCauley and the engineer take a space walk to repair the ship's engines. We hear their tools clanking against the metal hull as the repair is made, and they pound loudly on the hatch door to reboard the ship when they've finished. None of these sounds would be audible in the vacuum of space.

Scat, Darn Catwoman (2) - S2-E41
Audio problem: At the end, Catwoman falls from the roof into the water below. But we never hear a splash. (00:21:15)

Quantum Leap (1989)
Nuclear Family - October 26, 1962 - S3-E21
Audio problem: Just before the dog carries in the sack of shotgun shells, Mack and Sam have an argument, and Mack's dialogue is very poorly looped. Throughout the scene, the sound is out of sync with his lip movements. (00:33:00)

Hawaii Five-O (1968)
3,000 Crooked Miles To Honolulu - S4-E4
Audio problem: During McGarrett's second visit to question the professor, in the midst of their conversation, something off-camera makes a very loud thump. Professionals that they are, Buddy Ebsen and Jack Lord carry on with their lines as though nothing has happened. But this looks just a bit odd, because you wonder why their characters aren't curious about the noise. (00:34:10)

Batman (1966)
Audio problem: Joker's TV debut as an opera singer isn't very convincing. Even through the partially-obscuring clown mask, you can see that his lips aren't moving at all. (00:22:30)

Dark Shadows (1966)
Audio problem: 1897: Quentin challenges Barnabas by pulling a decorative sword from a wall display and pointing it at his throat. Unfortunately, the display wasn't all that well attached to the set wall. Though the camera pans away, you can hear a loud clunk as the piece falls, followed by the loud crash of shattering glass as it strikes and breaks a table lamp. You can also see the actors struggling very hard to keep straight faces as they continue with their lines.

Richard Diamond, Private Detective (1957)

Alias Smith and Jones (1971)
The Men That Corrupted Hadleyburg - S2-E18
Audio problem: Final sound edits were incomplete on this episode when star Pete Duel committed suicide, so his dialogue is "looped" by another actor in several places. Though it's an excellent imitation, it is most noticeably not Duel's voice when Heyes and Curry are waiting for Harry in the opening scene, at the campfire with the Tapscotts, and when Harry tells them about the trial's outcome.

Hawaii Five-O (1968)
Audio problem: Probably due to poor sound pick-up outdoors so near the ocean, much of the dialogue on the beach house patio is noticeably overdubbed. The sound is often out of sync with the actors' lips. (00:39:00)
1st Dec 2007
Hawaii Five-O (1968)
Kiss the Queen Goodbye - S2-E25
Audio problem: At the Governor's pageant, a group of young children sings and dances to ukulele music. But the song is just a little too noticeably dubbed in: none of the kids' lips are moving. (00:30:10)
29th Nov 2007
The Prisoner (1967)
The Girl Who Was Death - S1-E15
Audio problem: For some reason, "Londonderry Air" was dubbed over the original song being performed by the actors/singers. Their lips aren't moving in sync with the words at all.
