Revealing mistake: In Cameron's Story, in the scene with her and Derek, Derek's phone rings. When he looks at the screen it shows a telephone number and the words "EDIT" and "BACK" in the bottom corners. The phone is not in fact ringing at all. Derek is merely looking at the details of a contact saved on the phone.

Revealing mistake: When Catherine Weaver is killing all the employees of Desert Canyon Heat and Air in its warehouse, some of the employees fall to the ground with no signs of any entry wounds or blood on their bodies.

Revealing mistake: During the bar fight at the beginning of the episode, Mal gets knocked over and then hits one of the slavers with a chair. The chair barely touches him yet he still falls to the floor.

Revealing mistake: Look at the pilot in the cockpit of the fake ambulance (that the crew made to infiltrate St. Mary's hospital) as it lands at the main entrance. It looks nothing like Wash, not even lifelike.

Revealing mistake: During the firefight on Niska's ship, when Simon sees a guard coming and says to Sheppherd Book, "Book, we, uh-", a dead guard on the floor moves his legs as the guard that Simon saw fires his weapon. (00:37:10)

Wild At Heart - S4-E6

Revealing mistake: Towards the end when Oz says to Veruca, "You don't wanna find out what I am", and partially turns into a Werewolf. You can tell that both of the actors were filmed separately for the CGI's sake as Oz stands out from the picture more than Veruca does. The same effect is obvious during "Doppelgangland" in season 3 where there are 2 Willows onscreen at once.

Serenity (2005)

Revealing mistake: When River goes crazy on Serenity, knocks out Jayne and Simon and then holds Mal at gunpoint, you can tell that the gun she is holding is quite heavy because her arm wobbles. But when she cocks the gun it is completely still. In the cast commentary of the DVD, Joss Whedon reveals that the gun was in fact held up by a C-stand for that particular shot because the gun was too heavy for Summer Glau to hold. (Note: This is not the scene with River's choreographed fight in the bar on Miranda.) (01:02:45)

Serenity (2005)

Revealing mistake: On Miranda there is a shuttle crashed into the side of a building, but there are no burn marks or a trail from where it had crash landed. The shuttle couldn't possibly have crashed into the building horizontally without crashing diagonally/vertically first, landing horizontally and then sliding into the building. Joss Whedon even states in the director's commentary that some burn marks would have been nice but it only occurred to him about 14 months after it was filmed. (01:17:15)

The Big Bang - S5-E13

Revealing mistake: When the Doctor travels back in time after being shot by the stone Dalek, falls down the stairs and tells himself that he has 12 minutes to live we never see either of the 2 Doctors' faces onscreen at the same time, and if you look at the hair on the Doctor that's dying it is completely different to Matt Smith's. That's because it is Gordon Seed, Matt Smith's stunt double.

Bad Girls - S3-E14

Revealing mistake: Near the end of the episode when the Mayor releases the vampire from his cage and lets him cut his head in half, you can tell that the Mayor's head is a still image when it fixes itself. The expression on his face doesn't change at all and looks completely unrealistic.

Doppelgangland - S3-E16

Revealing mistake: Right at the beginning of the episode, Buffy is sat outside with Willow and Willow is levitating a pencil. The pencil is obviously CGI as it stands out from the rest of the picture and doesn't cast a shadow. It also disappears from view whenever there is a side-angle shot of Willow.

Doppelgangland - S3-E16

Revealing mistake: Near the end when the good Willow says goodbye to the evil Willow and hugs her, there is a shot of the back of the good Willow's head and the evil Willow's face. You can tell that the good Willow is a body double wearing a wig - there's something circular near her scalp, possibly an elastic band.

Revealing mistake: Both the good and evil Willows' faces are barely seen onscreen at the same time due to them being played by the same actress, but when they occasionally are the good Willow stands out from the picture more than the evil Willow. She must have been filmed separately from the good Willow and edited in later.
