Revealing mistake: Hannah sits in a conference room and pulls a landline phone towards her to dial her friend, asking about the will Owen left. The phone however isn't in any way connected, hence no call could have taken place.
Episode #1.6 - S1-E6
Continuity mistake: Sami takes Maya directly from the location where Tommy Dark's body is found to his home, but Maya's outfit is completely different when they arrive.
Factual error: At the very start, the lead parachutist says they're jumping from 30,000 feet, the altimeter confirms it, everyone whoops and cheers, and he gets them to put their masks on just before jumping. Two problems - the plane they're in is a De Havilland DHC-6 Twin Otter with a maximum altitude of 25,000 feet. And at 30,000 feet in an unpressurised aircraft there'd be nowhere near enough oxygen for shouting and cheering - they all should have had their masks on already.
Episode #1.4 - S1-E4
Continuity mistake: DC Li is wearing a white crewneck sweater with a blue band on the neckline until this episode, when she is suddenly wearing a blue button-up shirt. All episodes take place on a single flight.
Factual error: There is no way cops would just walk up and open fire on a suspect unannounced, not knowing if he is armed or not, and with an innocent bystander so close.
Caleidoscope - S2-E4
Character mistake: The chess set in Dr. Pierce's home office is set up incorrectly. When the men are set up properly, each player has a light colored square in the right-hand corner. The board as it appears in the show is set up so that the light-colored square is in each player's left-hand corner. This "beginner" mistake puts the king and queen in the wrong positions.
Other mistake: When we first see Gordon aboard his jet, the shot of the jet from the outside is a flipped shot. Note the lettering on the right engine.
Other mistake: When the interviewer asks the girl what she was doing March 15th, 2016, she says she was shopping. However when the receipts are shown they say 05/15/2016. (00:10:50)
Character mistake: Ryan tells Jack that Darius turned into a werewolf right in front of his and Micki's eyes. Ryan and Micki never witnessed Darius' transformation into a werewolf at all.
The Family - S1-E6
Character mistake: The pharmacist says she's sorry, but they don't have the drug Lamotrigine in stock, so would the generic be okay?Lamotrigine is already the generic form of the drug Lamictal. (00:28:23)
Revealing mistake: In brother Justin's padded cell, the far wall is obviously a painted backdrop. It even flutters when the orderly and doctor walk in! The only solid part is a couple of feet at the end, that Justin leans up against.
Factual error: A plastic bag of items is found under the footboard. Plastic bags weren't invented in 1969. They first appeared in public in 1979.
Visible crew/equipment: In episode 1, "Computer Logic", there's a scene in which Austin & Michelle step into a computer convention, where a hot-shot software designer, Mr, Blaine, is delivering a sales pitch from his wheelchair. In one shot the boom microphone can be seen dipping deeply into frame.
Witness for the Execution - S2-E15
Continuity mistake: When David and Maddie are debating whether or not to help Lawrence Everett, the towel David uses to wipe shaving cream off his face jumps around during the scene. It's around his neck, then in his hands, then on his shoulder, and back in his hands, etc. (00:15:40)
Continuity mistake: As Dave is leaning over the edge of the air boat with Rose behind him, he pulls a chunk of metal out of the water and places it on the edge of the boat on his left side. When he gets up from his lying position moments later, the metal is gone. Moments later again, with an overhead shot of the boat, we see what appears to be that piece of metal positioned at what would have been his right side, not his left. This is also where we see him step over the seat (twice!) in subsequent camera angles. (00:30:00)
Factual error: At the beginning of the episode, Patrick Swayze is looking at photos of two Iraq veterans who have been murdered. He identifies one as a Marine Lance Corporal and the other as an Army PFC, but both pictures show men in U.S. Army uniforms. A bit later, his partner, who is supposed to be a Marine vet, refers to his decision not to "re-up," using the Army term for re-enlistment, rather than "ship over," as it's referred to in the Navy and Marines.
Plot hole: During Ryder's report, as Bruce and company watch it on TV, the camera suddenly zooms in on Joker standing on a catwalk above Ryder, and none of the crew, especially not the camera man who caught the villain, makes a comment about this. They may have thought the Joker (or rather an impersonator) was perhaps a surprise gag in the show, but since this is supposed to be a serious documentary report, it is still strange that they wouldn't point it out.
Episode One - S2-E1
Factual error: During this episode, Vincent is remanded to prison for suspected murder. At one scene he is seen working on the wing. Remand prisoners are not expected or allowed to carry out work duties in British Prisons.
Testimony of Evil (Dead Men Don't Laugh) - S1-E6
Continuity mistake: At the start, the glass on the left is poisoned. The guy picks up the glass on the right, but still ends up dying.
Continuity mistake: When Mary talks to Bobby D at the crime scene, her hair moves back and forth over her left shoulder between shots throughout their conversation.
