Visible crew/equipment: Magnum and TC are hurrying to catch an elevator in the parking garage. As they approach the elevator, you can see a cameraman against the wall on the right side that should not be there. The next shot shows the scene from that cameraman's perspective. (00:39:30)

Visible crew/equipment: When Jim Greer is talking to Elizabeth Wright where she finds out Jim undermined the mission, you can see a member of the film crew holding a camera on his right shoulder in the reflection of his sunglasses. (00:43:05)

Visible crew/equipment: As the team drives through Mumbai heading down the tunnel you can see the camera man for a brief moment behind the gunner atop the truck.
The Blessing of Many Fractures - S1-E8

Visible crew/equipment: After Jax cuts the rope that's supposedly keeping the trap in the air, as Marty falls to the ground you can see the cable that is actually holding the whole contraption up while the rope falls a bit to the right.
A.K.A I Have No Spleen - S3-E3
Visible crew/equipment: Jeri Hogarth and her ex Kith Lyonne are alone in Hogarth's apartment. Lyonne takes her cello and walks across the room. The next shot is of Jeri. To Jeri's right, a crew member is crouching in the shadows behind the room divider. As the shot continues, the crew members stands up. (00:38:30)
Visible crew/equipment: When Winn is putting on his shirt after being caught, you can see his mic wire through the hole where Kara says he forgot to button his shirt. (00:03:15)
Blade VS Buffy Summers - S12-E10
Visible crew/equipment: After kicking Buffy away and she hits a smiley face on the wall, Blade jumps at her and kicks off the wall to kick her again. If you look closely at the left side of the screen where the bathroom mirror is, you can briefly see a camera for a couple of frames filming the action just on the edge of the frame. The lighting in the bathroom makes it sort of hard to see. (00:21:16)

Visible crew/equipment: The microphone is visible at the top of the screen when Guy confronts Marion back at Nottingham castle. (00:29:20)
The Greatest American Hero (1) - S1-E1
Visible crew/equipment: When Ralph joins hungover Bill in the boys bathroom, in some shots the boom mic casts two moving shadows on the door and wall, at the top of the screen.
Visible crew/equipment: As Blue escapes from the fake Cloudbase, a puppeteer's arm can be seen throwing the puppet through the window.
Visible crew/equipment: When Sydney taps the shoulder of the baddie who nearly disarmed Nigel, a strong light is cast on his face just before the cut. Nobody was supposed to point a flashlight or a torch at the fight. (00:38:15)
Visible crew/equipment: In the live-action segment, several times when Mario and Luigi confront Mrs. Gammliss, the shadow of either a crew member or a piece of equipment is moving at the top of the screen. It is not Mario, Luigi, or the Ratagator's, as they are not moving, and it is not the passersby's, as the shadow is present even when they are not present. (00:17:38 - 00:18:45)
