Let the Games Begin: Part Two - S5-E4

Character mistake: Professor Hawkins says that there are seven cards required to summon Ancient Dragon and also mentions that until quite recently, both he and Solomon had six of the seven cards. But in the flashback of Solomon speaking to Hawkins at the airport, he tells him he managed to track down the fifth card, even though there were seven cards they were looking for. Duel Identity, Part II - S1-E30

Character mistake: Yugi mistakenly says if he doesn't get a monster out to defend himself Mai can attack him directly, which wouldn't be possible since the rules of Duel Monsters during the Duelist Kingdom arc forbade direct attacks on defenseless opponents. That rule wouldn't be implemented until the Battle City arc.
