Continuity mistake: We can see Jamie driving a Ford CVPI with plates reading 7554, however when he appears in the following shot in the court, where he was going to in the previous scene, he's driving a Chevy Impala with plates 61795. I am not sure, but I think this error happened in the previous seasons as well. (00:09:08 - 00:09:48)

Visible crew/equipment: While Frank visits with the Cardinal to tell him about the time sensitive situation, when Frank asks about St. Mary's surveillance cameras as they both sit down, the small green T-mark is visible on the rug under the Cardinal's chair. (00:23:40)
Character mistake: Commissioner Frank Reagan refers to Lt. Gormley as Sgt. Gormley in this episode. (00:08:30)
Continuity mistake: Part way into the episode they show the front of Jamie's Ford police car and the car number on the front plate, but when they pull up to the court house to talk to his sister, it's a Chevrolet police car with a different number on the front. (00:09:05 - 00:09:40)
The One That Got Away - S7-E13
Visible crew/equipment: After Eddie tells Jamie that she wanted Josh to swoon over her like Caroline seems to be doing with Jamie, when it cuts to Erin as she tells Danny that Child Services will likely release the boy to his parents, as Danny says, "Hold on," the green T-mark is visible on the floor. (00:22:30)
In and Out - S7-E14
Visible crew/equipment: While Danny and Baez are hiding in the hall, when Ramirez (parole officer) knocks on Lloyd 's door he gets the gun from under the seat cushion, then as Lloyd starts to walk to the door, the green T-mark is visible on the floor near the right side of the screen. (00:34:20)
In and Out - S7-E14
Continuity mistake: At the start, the rearview mirror on the windshield of Eddie and Jamie's squad car disappears and reappears.
Visible crew/equipment: After the opening credits, when it cuts to Frank's office a red tape mark is visible on the floor near the window, at the right side of the screen. After Nicky leaves the office it appears Frank walks to that area to look out of the window. (00:04:35)
Continuity mistake: When Danny and Jimmy drive up to D'Angelo, the rearview mirror appears, disappears, and reappears on the windshield (though its small square adhesive is intact). (00:28:30)
Visible crew/equipment: After Danny tells Linda all that matters is her brother is alive and her family is safe, when it cuts to the courtroom there's an overhead shot from behind Frank, and the actor's yellow tape mark is visible on the floor at the left side of the screen. (00:34:20)
Visible crew/equipment: Towards The End of the episode as ADA Reagan is walking to her car, you can see the cones in the DAR end of the street the filming crews used for traffic control to keep traffic off the set. (00:02:30)
Continuity mistake: When Danny/ Baez and Jamie go to arrest the medic, there are a few different shots where there is snow on the ground and then none, likewise no snow on cars and then a lot. (00:08:00)
Visible crew/equipment: After Baez tells Danny that detectives from Moscow are on their way to help with the investigation, when it cuts to Anthony entering the courtroom, just as Erin removes her glasses the T-mark is visible on the floor behind her, at the bottom left side of the screen. (00:08:35)
Visible crew/equipment: When Danny and Baez are at the scene of the firebombing while it's raining, in the second shot as they walk toward the eleven body bags, three nylon T-markers (yellow, red, and green) are visible on the wet ground where Baez, Danny, and Molina will be standing in the following shots. (00:02:45)
Visible crew/equipment: After Anthony drives the Porsche 911 into the garage, in the shot facing Erin and Anthony just as Jimmy gets into the car, the green T-mark, the red tape mark, and blue tape mark are visible on the floor.
Continuity mistake: While Danny and Baez are questioning Damian about Mason's whereabouts, Damian's right handcuff switches to the opposite way depending on the angle of the shots. Either the single metal bar faces up or the double bar faces up, which is quite impossible to change when it's locked around his wrist.
Other mistake: Jamie races to the patrol car after witnessing a car jacking with possible child inside. He flings the door open, gets halfway inside, and the car starts without him even being close to turning the key in the ignition. (00:04:00)
Other mistake: Jamie and Eddie respond to a carjacking of a minivan. They are across the street and Eddie claims there's a baby seat in the car. There's no possible way that she could see if a baby seat was in the car. The minivan had dark tinted windows and only the driver's door was open. (00:05:00)
Factual error: Danny shoots Frank's revolver, and as he continues to shoot, you hear the sound of a casing hitting the floor, which shouldn't happen since it's a revolver being fired. (00:11:46)
Audio problem: While at the range, Jamie takes Frank's revolver to the line and fires it. The last 2 shots you can hear the casings hitting the ground, but not on the first 3 shots. The last 2 shots are from another camera angle. Revolvers don't eject the casings, so we shouldn't hear them landing.
