Factual error: The panzer caps worn by the German tankers (and later by Saunders and Gavin) are old-style pre- and early-style caps that were no longer issued later in the war.
Other mistake: The view from tank driver Gavin's periscopes shows he sees an approaching German tank, but his tank has nosedived so far into the hole his tank is stuck in that there is no way he could have seen anything except a wall of earth and rocks.
Revealing mistake: The grenade Saunders tosses onto the German tank does not land where the pyrotechnics explode.
Other mistake: When Saunders' tank and the other tank are sparring against each other, closeup shots of the tanks' rear drive sprockets show them moving in reverse, but overhead shots of the vehicles show the tanks moving forward.
Continuity mistake: During the opening scenes of the German attack, a shot of a B.A.R. gunner being shot is used three times.
Main Event - S4-E1
Continuity mistake: Saunders' helmet disappears from his head between shots when he's driving the half-track.
Factual error: As Stevens charges the German machine gun nest, he fires more bullets than his M-1 rifle could hold (the rifle could hold only eight rounds per clip).
Continuity mistake: As Sgt. Saunders and Pvt. Stevens are about to enter the HQ, in the close up shots are several soldiers walking out of the HQ towards them. In the long shot of Saunders and Stevens the soldiers have disappeared.
The Casket - S4-E12
Continuity mistake: Saunders steals 1 of 2 gas cans, mounted on brackets, from the side of a German truck. In later scenes, the remaining gas can and the brackets are gone.
The Raider - S4-E16
Continuity mistake: During The Gun battle at the farmhouse, the number of bullet holes striking the window frame varies from scene to scene. Also, the window frame gets shot to pieces, but in subsequent scenes the window frame is intact-aside from the bullet holes in it.

Audio problem: When the jeep peels out on the grass, it makes a screeching tires-on-asphalt sound.
Factual error: As Caje provides cover fire for Sgt. Saunders, Caje's M-1 rifle fires more than the eight shots it holds.
Revealing mistake: After Marsini dies, he is still breathing.
Continuity mistake: Sal Mineo and Tom Skerritt's characters are fighting in a dark room that is suddenly brightly lit after the commercial break, even though their fight is continuous and no one else is there to turn on any lights.
The Ringer - S4-E23
Continuity mistake: Caje extinguishes the fire in the farmhouse, but a few shots later, it's still burning. A little later still, it's out again.
Hills Are for Heroes: Part 1 - S4-E25
Continuity mistake: After Lt. Hanley says that the machine-gun needs to be set up closer and turns to go into the bunker, he is wearing a backpack. When he enters the bunker the backpack is gone. (00:04:30)
Continuity mistake: The demolition expert is shot and falls dead several feet from Hanley's position. Next shot, his body is now right beside Hanley.
Revealing mistake: Blank .30 caliber machine gun bullets are obvious as the soldier with Little John sets up and loads the weapon.
Continuity mistake: Long shots of the German sergeant shows his MP38/40 submachine gun at his waist and pointed down, but the close up shots of him show the weapon up by his chest, pointed up.
Revealing mistake: The German MP38/40 submachineguns have blank firing adapters on their barrels.
