Factual error: When browsing for information about the crow deaths in the Ganwar Region of Somalia, the computer indicates that the CDC requested additional funding from DHS. The only problem with that is, DHS was formed in 2002, 11 years after the incident in Ganwar. (00:40:05)
Suggested correction: Although the Department of Homeland Security - commonly abbreviated as DHS - was not formed until 2002, in the later episode "Revelation Zero" it is indicated that DHS stands for an agency called Demographic and Health Services.

Factual error: There is a scene which shows the Blackbird population as a graph on the computer monitor. On the day of the FlashForward event, this graph shows the Blackbird population dying off in October, but there is a mistake because the graph also shows "November" and "December" values which did not yet occur.
Continuity mistake: When the girl working in the hospital sees a man from her flash forward, she runs after him carrying several hospital client files. As she runs through the hospital stopping people, the files magically disappear from her arms.
Audio problem: About 16 minutes into the episode, Wedeck is on the phone with his wife talking about how he hates DC. One of the Senators from the hearing walks up to Wedeck and asks, "Are you enjoying DC, Stanford?" Wedeck say (to his wife on the phone), "I'm gonna have to call you back," but his mouth doesn't move with the words. The actor flicks his tongue out while the audio of him saying "I'm gonna have to call you back" plays.
Factual error: In Episode 10, A561984, Simon asks for the image of the Somalia terrain to be enlarged, and when he sees the image of the Specialised Laser for a Pulse Afterburner, his attitude suggests that he expected to see it on the Solmalian landscape. But then talks about it being in its design stage, and is surprised when Janice reveals that it is already a real structure. (00:18:10)

Other mistake: At the beginning of the scene on the plane, Zoey is sending a text message to Demetri. When her phone is in the shot, what is shown on its display screen is obviously edited, because she makes the mistake of pressing the "end call" instead of the "send call" button to send the message.
