Deliberate mistake: Pre Vizsla's appearance in the hologram doesn't match up with his appearance in the previous and succeeding episodes, as he's wearing clone armour covered with a cloak that has a different symbol on it. Continuity mistake: At the beginning of the fight with the assassin droids, Anakin cuts off the right arm of the one Ahsoka ends up fighting. Yet during the fight, just before she says "Get back, ya pistonhead", the droid briefly has both arms. (00:12:10)

Continuity mistake: Chopper's eyes are different colours, with his right eye a whitish hue. However, in the close-up of him after he first comes under suspicion, both of his eyes are brown. (00:12:02)
Answer: I would say his death in Phantom Menace was more of a controversy. He was a fan favorite character from the movie, and his fight scene largely regarded as the best part of the movie. It was more he was brought back to please the Star Wars fans and few, if any, had problems with it.
