Other mistake: The killer from another dimension shape shifts into the appearance of the victim, leaving the victim's body. But it transforms into Charlie's appearance and leaves behind its former female nurse shape, hiding Charlie's body to dispose of later. It would have needed to transform from the nurse shape, not out of nothing. The original nurse's body was not at that location.

Suggested correction: The shapeshifter transformed into Charlie using his dead body which it then disposes of at the end of the episode.

But when the shapeshifter transforms into Charlie, he shouldn't be leaving the body of the nurse behind since he had killed the nurse before entering the hospital and transformed there into her. The only dead body in that last scene should be Charlie's.

A New Day in the Old Town - S2-E1

Other mistake: Just after Olivia wakes up in hospital, she's trying to explain what happened, and we hear a heart monitor getting faster and faster - the doctor looks concerned and says they need to get her pulse down. But when we cut to the monitor itself it shows her heart rate as 72 - a perfectly normal value.

And Those We Left Behind - S4-E6

Plot hole: The very ending scene where Peter (in this version died in the ice as a boy so Walter's wife never met him) takes off some of the dust covers then picks up a toy airplane. The toy airplane he got as a child from ... Walter's wife when she was trying to placate him in an earlier episode for trying to run "home". This toy airplane wouldn't have been his as a child in this version of the universe. (00:42:00)

Question: In the original timeline, when Fauxlivia and our Olivia are in their opposite universes, the thing that makes our Olivia remember her real life and try to escape is the memory of Peter. In the new timeline, he doesn't exist. When Walternate has found and synthesised the Cortexiphan, he says that our Olivia is no longer necessary and to kill her. How did she get back to our universe with no memories of Peter, and no alternate Broyles helping her to escape and being sent to us dead with one leg?

Answer: In the time line of Season 4, William Bell is trying to destroy both universes to create a new one, and he needs Olivia to power the collapse. So he helped her escape. But the real question is, if not in search of Peter, why Olivia has to travel to the alternate universe to be captured and replaced?
