Continuity mistake: Charlie, Olivia's former partner and friend in the FBI shows up in this episode during the interview with Mrs. Rusk, but Charlie was killed off by one of the shapeshifters in a previous episode, and the shapeshifter that killed Charlie was himself killed off by Olivia, who in other episodes reflected back on killing him. This episode was supposedly meant to be in season 1 as ep 21 or 22. Unknown why they didn't air it then, and decided to put it in the middle of season 2 (for no apparent reason).
Continuity mistake: The bullet hole in Davidson's chest is not at all where he was shot.
Continuity mistake: When Walter tells Peter that the Cocabeans he bought can't be eaten, he takes the roasting pan off the bunsen burner. In the next shot, the pan is still on the burner. (00:06:55)

Continuity mistake: This is when Olivia goes to visit the boy at the hospital the second time. She and the guy from Department of Social Services go outside to talk. One of the shots is flipped. Note Olivia's notepad and the heart monitor by his bed. (00:19:40)
Continuity mistake: At the very end of the episode when Nina Sharp exits the elevator, she mostly steps out of the elevator completely before the masked men come and shoot her. However, when they review the security footage of the attack in the next episode, "There's More Than One of Everything," you see that she barely takes a step before being shot. (00:49:15)
And Those We Left Behind - S4-E6
Continuity mistake: While beginning to take the radiation readings of the Jeep during his time jumps, Peter has a wedding ring on his finger. After cutting away and back again, the wedding ring is no longer on his finger. (00:15:15)
Continuity mistake: This episode takes place over one day. It's Olivia's birthday at the start of the episode, and at night, she gets the card from her stepfather, implying that it's still the same day. Right before Broyles yells at Olivia in the Harvard halls, there is an establishing shot of Harvard where it is nighttime outside, but inside the building, there's sunlight coming through the windows.
Continuity mistake: On the park bench, while talking to Peter, Olivia's hair moves back and forth over her right shoulder. (00:32:40)
Night of Desirable Objects - S2-E2
Continuity mistake: A newspaper death notice of a mother and child is shown dated "September 1992". Two scenes later the tombstones of the same individuals are shown, dated "November, 1991"
Continuity mistake: After Fauxlivia shoots Joe and is dragging him to the bathroom to dispose of the body, she hears a knock on the door. During this shot, she is dragging him with his jacket over his head, which she is gripping. However in the immediate next shot, she is still dragging him by his jacket, only the jacket is much lower.
Continuity mistake: Right at the very end, Olivia comes to meet Peter at his place for drinks. While Peter goes to grab his coat, her hair moves behind her right shoulder in a consecutive shot.
Continuity mistake: When Walter is using the hammer on the block of ice, the pattern that results is different when viewed from each side.
Continuity mistake: The front license plate on John's SUV falls off during the chase with Olivia. It reappears seconds later. (01:15:30)
Continuity mistake: When Olivia crosses over to the other side you see people wearing FBI jackets. But according to later episodes that had changed to Fringe Division on the other side years earlier.
Brave New World: Part 1 - S4-E21
Continuity mistake: This is when Peter and Olivia are discussing different Boston's suburbs as their potential new home. The sun is on the right side of Olivia's face in one shot. In other shots, it isn't.
Continuity mistake: At the club, when Peter and Olivia are looking for Valeria, Peter is wearing an ear piece in his left ear. When he talks to the woman, the piece is missing.
Night of Desirable Objects - S2-E2
Continuity mistake: Just before Walter's hand goes numb, Jean the cow can be seen alone in her pen. Then in the next shot, as Peter walks behind his father, Astrid has instantly appeared in the pen with Jean, brushing her. (00:13:40)

Continuity mistake: The left side of Peter's face is alternately bathed in sunlight or not throughout the scene when he and Walter talk in front of Olivia's hospital room.
Inner Child - S1-E15
Continuity mistake: Olivia, Broyles, Peter and Walter are visiting the boy for the first time at the hospital. A small bit of Olivia's hair is in front of her left shoulder. Cut to the immediate next shot from the patient's room and all of her hair is behind of shoulder. (00:07:05)
Continuity mistake: During one of the first shots of Olivia in the tank, you can see the reflections of lab lights on the water even though the tank is closed.
Answer: In the time line of Season 4, William Bell is trying to destroy both universes to create a new one, and he needs Olivia to power the collapse. So he helped her escape. But the real question is, if not in search of Peter, why Olivia has to travel to the alternate universe to be captured and replaced?
