The Lighter-Than-Air Man - S5-E8
Visible crew/equipment: When all the guys are on site of an overturned tractor trailer, Captain Stanley says "Let's cool off that cab." In the next closeup of Stoker spinning a gear, the reflection of the cameraman (with mustache and hat) holding a handheld camera is visible on the engine's red surface.
Visible crew/equipment: When Engine and Squad 51 are at the cliff with the young unconscious boy, Captain Stanley calls in to dispatch, with Stoker standing beside him. The reflection of the camera crane and location set lights are visible in the side door mirror.
Pressure 165 - S5-E7
Other mistake: When Johnny prepares Chef Michel's recipe, he leaves the casserole dish in the oven while Station 51 is dispatched to the structure fire. When he and Roy return, Johnny opens the hot oven door to find it smoking and the dish bubbled over, and even though the oven door is supposedly very hot, Roy places his hand on the inside of that door and actually leans on it like there's absolutely no problem.
Visible crew/equipment: When Roy and Johnny leave Rampart and are heading back in the squad, about 16:40 minutes in, as they are discussing whether Roy has told his wife about house hunting, you can see two signs on the ceiling of the squad's cab. The signs say 'Emergency Saturday 8PM Rehearsal Mark VI Prod'. Possibly they are magnetic signs for the squad doors so people don't mistake the squad for a real fire department vehicle? (00:16:20)
Visible crew/equipment: When Dixie tells the attendants to wheel Ruthie, the woman who went berserk at the salon, into the treatment room where Dr. Early's waiting, there are two T-marks visible on the floor at Dixie's and Johnny's feet.
Continuity mistake: When Johnny and Roy are at the apartment building with the broken elevator, Roy's watch is gone when they climb the steps, but he's wearing the watch when they enter the apartment, then it vanishes when they head back down. That watch reappears again, disappears and reappears until they go back up to the apartment for the second time.
Visible crew/equipment: While rescuing the invalid, when Roy climbs out of the second floor window he suffers an electrical shock, and when he falls in the closeup we can see the crash mat that he lands on when part of it flips up from the force of his weight. Also note that all the shrubs below the window have vanished.
Visible crew/equipment: While Brackett is on Copter 10, after the dispatcher tells the pilot that the truck driver has been heard on the CB saying that he's trapped in the cab, repeating the same location, when we see the bird's-eye view of Copter 10 flying below, the shadow of the filming helicopter is right beside the shadow of the Copter 10 chopper, at the top of the screen. (00:11:35)
Visible crew/equipment: After Station 51 is dispatched to the fire at the marine salvage yard, when they arrive at the scene we can see a second cameraman on his knees beside the camera, as he is shooting footage of the ship from that specific angle, which we see during this scene.
Visible crew/equipment: After the guys rescue the woman who was trapped upside down in the wrecked car, when they load her into the ambulance, the location set lighting is reflected on the glass of the ambulance doors.
Visible crew/equipment: After Captain Stanley tells the guys about the doctor who's doing the diet/nutrition study, when they leave to check the new refinery hydrants, the actor's T-mark is visible on the floor where DeSoto had been standing. Also note that DeSoto is wearing slippers when he stands up. It happens again later, he's wearing slippers with no socks when Morton is talking to the guys about stress and diet.
Visible crew/equipment: When the guys are looking at the vitamins that Chet brought for them 51's tones drop, and as they leave the kitchen the two actors' T-marks are visible on the floor where Mike and Marco had been standing.
The Tycoons - S5-E23
Visible crew/equipment: After the alley's dumpster fire has been put out by Phyllis, when Captain Stanley tells Marco to wash it down, two tape marks and one T-mark are all visible on the ground.
The Tycoons - S5-E23
Visible crew/equipment: At the chemical plant fire all the plant personnel are evacuated. When Gage yells that one of the valves blew, and that he's covered in the flammable pentane, the camera spins around and a crew member is visible on the right side of the screen, just as Captain Stanley and DeSoto walk up and pass him.
The Tycoons - S5-E23
Visible crew/equipment: At the chemical plant fire, when Gage is covered in the flammable pentane, Chet hoses him down. When Marco says "John, you better stay here, " Chet and Marco head off. In the next shot, when the camera pans quickly to the left of the blaze, one crew member wearing jeans and a white shirt stands behind the firemen, and another crew member wearing a grey shirt crouches at the side.
Visible crew/equipment: When Gage and DeSoto are on their first run (the mother-in-law), they phone Rampart's base station. When Dixie opens the door to answer the call, her mark (strip of black tape) is visible on the floor just as she steps on it.
Continuity mistake: When Johnny and Roy are with the heart transplant patient who is having chest pains, after they contact Rampart, in the wide shots Johnny has the stethoscope around his neck, but in his closeups the stethoscope is gone.
Visible crew/equipment: When Gage and DeSoto are stuck cleaning the squad again before The Inspection, Captain Stanley walks in and the actor's T-mark is visible on the floor just before he steps on it. Additionally, when Stanley walks away, and Chet catches up to him, another T-mark is visible on the floor near the doorway.
Visible crew/equipment: After Chet is brought to the ER, Gage goes to the nurses' station. When Brackett opens the door and walks out of the base station booth, the actor's T-mark is visible on the floor behind him.
Visible crew/equipment: When Brackett's helicopter takes off the filming helicopter can be seen through the window. (00:03:45)
Answer: I was rewatching a few first season episodes of Charlie's Angels (1976), and in S1xE6 "The Killing Kind," I recognized the same actor. So, to finally fully answer your question, the two baseball players in School Days are played by Rod Perry and Sean Fallon Walsh.
