Continuity mistake: Series 2 - Episode 10. When Sara is to meet Michael at 1681 Butterfield Road Sara looks in the mailbox. She closes the lid, yet the mailbox is seen standing with the lid a bit open. In the next couple of shots the mailbox is completely open, and when they jump in the car and leave, the lid is now closed.

Continuity mistake: All throughout the car chase, the car Michael and Lincoln are in has a license plate on the front bumper. But after Bellick has forced their car off the road, the car makes a jump before hitting a tree, and while it is in mid-air you can see that the license plate has disappeared. (00:02:50)

Riots, Drills and the Devil: Part 2 - S1-E7

Continuity mistake: This happens during LJ's fight with his mom and step-dad. When LJ says "You got no clue do you?" you can see Lisa's hands on his shoulders trying to calm him down. When the shot changes, Lisa's hands are on her hips. (00:18:45)

English, Fitz or Percy - S1-E5

Continuity mistake: When Michael is supposed to be in Pope's office holding the support for the Taj Mahal so it didn't come unglued, Abruzzi placed the makeshift key in the lock of the door, allowing Michael to escape to get back with the other inmates. When Michael opens the door and it's unlocked, The Key is in the deadbolt. As he shuts the door behind him, The Key is in the lock of the door knob. (00:23:20)

Continuity mistake: In the scene in Panama in the last episode Michael is on the boat and a boy comes along with some sodas. In that scene the sky is overcast. But in the next clip, when the boat is seen further away the sky is clear.

Continuity mistake: When Scofield tries to make Haywire remember the path, he touches his chin with both of his hands, and there are no tattoos on his left arm. (00:10:25)

First Down - S2-E4

Continuity mistake: When agent Mahone is looking at the photocopied newspaper articles, at first he is holding a single sheet of paper, and you can read the headline in reverse through the paper: "Life Sentence for Mob Boss Abruzzi". We then get a close-up of the articles from his point of view, and he is now holding two sheets of paper, where the topmost reads "Governor's Daughter Wins Humanitarian award". Mahone places this paper on his desk, and you can see that he is once again only holding the one "Life Sentence." paper he had to begin with. (00:05:30)

First Down - S2-E4

Continuity mistake: Bellick's partner, Geary, is wearing a light grey T-shirt, with a very prominent sweat stain on the front. Throughout the episode, this stain constantly changes shape and size, even within two shots of the same scene, and sometimes it disappears altogether. This is best seen when him and Bellick are herding the prisoners to their car. In the close-ups of him, his shirt is completely dry, in long shots the sweat stain is there. (00:04:45)

Manhunt - S2-E1

Continuity mistake: When Lincoln and Michael are talking on the little pier, Michael talks about Westmoreland and his relationship to the truth. Lincoln has his head turned towards Michael through this, but as Michael says "There'd be no reason for him to lie about it", the angle changes and Lincoln is suddenly staring straight ahead, with no time to turn his head between shots.

The Old Head - S1-E8

Continuity mistake: In the scene where LJ comes to visit Lincoln and admits to breaking the coffee table, LJ's hand moves between shots. When Lincoln is shown, you can see LJ sitting in front of him with his chin resting on his hand. When the shot changes to close-ups of LJ, his hand is down and completely out of the shot. (00:08:10)

Continuity mistake: Episode 14 - John Doe. Bellick is in the prison, and is being threatened by 'Banks.' As Banks and his crew walk away, in two different shots, you can see an elderly gentlemen (looking like Charles Westmoreland) walk across the screen, left to right. He does this twice, without there being time for him to run around.

The Rat - S1-E14

Continuity mistake: In Episode 14, "The Rat", when Lincoln and Dr. Tancredi are in his cell together, he says "How it gonna feel when they tighten the belts". He then turns his head so that he is looking the doctor in the eyes, but when the angle changes, he is staring directly in front of himself, and his head is not turned at all.

Continuity mistake: When Steadman takes out his gun, he presses it to the back of Veronica's head. She does not flinch or shy away from the gun, but in the next shot, a close-up of Steadman, the gun muzzle is pointed at the camera with Veronica off-screen, and so the gun is suddenly not touching her. (00:22:40)

And Then There Were 7 - S1-E11

Continuity mistake: When Quinn falls in the well, he falls through and subsequently destroys the well cover. At the end of the episode when the Secret Service boys show up, an undamaged cover is lying next to the well, which Kellerman uses to cover it up. (00:29:30 - 00:38:30)

Greatness Achieved - S4-E9

Question: How does Don Self know that Miriam should work at Gate when nobody knew about the location of Scylla? Or Whistler wanting to work there?

Answer: Don Self actually worked with Whistler before he died. And Whistler with the bird book actually knew where Scylla was.

Answer: Because Miriam was an agent.

But that still doesn't make sense because nobody knew where Scylla was.

He knew that GATE was mentioned in the bird book. He worked with Whistler and had probably discussed GATE with him, so it was clear enough that it was a big part of the plan to get Scylla. Sending an agent to work there in any position would've been a big help for intel, as she could assess the environment and perhaps uncover what Whistler's plan was. He didn't anticipate T-bag's hijacking of the plan, hence why he sent Miriam and not a more combat-trained agent. She was only there for information.
