Continuity mistake: Laura fires at the wall, leaving a bullet hole near Waverly's head. In the next shot, as they're effecting their escape, the bullet hole has disappeared. (00:44:05)
Continuity mistake: Solo and Illya park their big Chrysler convertible in front of Del Floria's and go inside. The shot then cuts to the tailor shop's interior with a view of the street through the window, and the car they just parked out front is nowhere to be seen. (00:00:20)
The Project Strigas Affair - S1-E9
Other mistake: Vladik, incorrectly, tells Donfield on the phone to stand at the hot dog stand at 3 pm Monday. It should have been 11 am. Donfield in the "meet" scene refers to his 11 o'clock feeding" and Mr. Smith tells Solo (Donfield) his next meeting is between one and two pm. This would be impossible if the original meet was at 3 pm.
The Summit-Five Affair - S4-E1
Continuity mistake: When Napoleon and Illya are trying to escape from the steam room, and Illya accidentally drops the key to the door, Napoleon points to where he thinks the key is with his right hand, with his left hand resting on the bench above him. When they finally find the key, Napoleon's left hand is pointing at its location. (00:40:25)
The Apple A Day Affair - S3-E27
Continuity mistake: When Napoleon and Illya escape from the mine and chase after the Thrush truck, it is dark outside. For one shot of Napoleon and Illya in their car, it's very obviously daytime behind them. (00:46:20)

Audio problem: When Parviz realizes Napoleon stole his gun, his mouth doesn't move when he says "My pistol." (00:20:35)

Continuity mistake: When Napoleon and Samoy are wondering if Thrush poisoned Waverly in order to deliver him to a specific medical clinic, the mannequin hand Solo has resting on Samoy's shoulder disappears between shots. (00:21:00)
The Mad, Mad, Tea Party Affair - S1-E18
Revealing mistake: When Riley is trying to escape after being exposed as the saboteur, he shoots a pair of U.N.C.L.E. agents. On the right side of the screen, spent shell casings are very obviously thrown into the shot from off screen. (00:43:40)
Answer: He must have ridden in an Eaton's store elevator while he was in Canada at some point. For some reason, this elevator reminds him of it. The show's original concept had Solo being a Canadian, so this may be a minor nod to that fact.
