Continuity mistake: At the start, the disk falls out the paper and the case is touching the door mat. When the shot changes to a close up of the case being picked up, the case is no longer touching the mat. (00:01:05)
Continuity mistake: Ziva is photographing the scene and takes a picture of the empty DVD player with a yellow evidence marker number 5 on top of it, Gibbs is then seen with the same marker in his hand before placing it on the DVD player. (00:04:40)
Continuity mistake: When Ziva is photographing the bedroom she has the camera raised to her eye in one shot and lowered in the next when the DA asks about DiNozzo. (00:05:40)
Continuity mistake: When The DA comes into NCIS after confronting Gibbs at his house, Ziva's action's in the background are constantly changing between cuts. (00:23:40)
Visible crew/equipment: When Gibbs tells Gail Walsh that they're keeping Jerry at NCIS, Gail replies, "Only on the condition that you keep me apprised of any new information," and in the next shot from behind Gail, a crew member (presumably the boom operator) wearing a black shirt and white pants is visible at the left side of the screen. Other mistake: McGee says that Jancy got into her cab at 12:37am and arrived at her destination at 12:55. That's 18 minutes for the taxi to drive "over 20 miles away" Hardly likely given traffic. (00:29:00)
Suggested correction: You missed the point of what he said. The cab ride ends at Gilroy's address, which is 20 miles from Travis Wooten's home. But the ride didn't start at Wooten's home, it started at Feeney's Billiard Club. They don't make any mention how many miles the ride was.
A Man Walks Into a Bar... - S8-E14
Visible crew/equipment: While Gibbs and Tony are in the galley interrogating the two members of the crew because they worked closely with Commander Reynolds, when Gibbs asks them if the gun looks familia, the boom mic dips into view at the top of the screen. (00:32:40)
Kill Screen - S8-E16
Visible crew/equipment: When McGee and Blake Martin are in Interrogation, in the shot facing McGee just as Gibbs sits down, the reflection of the camera operator is visible in the mirror, on the right side of the screen. (00:33:30)
Visible crew/equipment: When Gibbs finds out that Otto has been found dead, he looks at Leona. In Leona's glasses you can see the reflection of the filming crew. (00:30:50)
Visible crew/equipment: After Tony says the line "What would Steve McQueen do?" we get an extreme closeup of the front bumper of a car with Virginia plates on it. Just to the screen right of the plate, you can see the reflection of some of the filming rig used for the shot. (00:31:50)

Continuity mistake: The way the axe faces when Leon leans it against The Wall changes between shots. Sometimes it's facing left, others it's facing right. (00:28:00)
