Continuity mistake: When the Doctor starts walking over to the lakeshore after the astronaut appears, he's still holding the wine bottle in his right hand. In the next shot where we can see his hand, the bottle is gone, without him having stopped to put it down. (00:06:34)
Continuity mistake: When Gadget is driven to the TARDIS so the Doctor can retrieve it, the landscape around it is much flatter and without the large boulders strewn around the TARDIS when the Doctor arrived on Mars at the beginning. (00:52:41)
Continuity mistake: When the Doctor leans on the railing above the glacier while telling Adelaide about the Ice Warriors, he rests his left hand a ways out on the railing, in a spot where it should be visible but isn't in the next shot from behind him. (00:28:57)

Continuity mistake: After questioning the houseguests, the Doctor and Agatha are talking. She uses the phrase "little grey cells", and the Doctor responds, "Oh, yes, little grey cells. Good old Poirot." When he does this, he's holding a saucer in his left hand and rests a teacup he's holding in his right hand atop it, as he turns toward the chair that the suspects were sitting in. In the next shot, as he walks over and sits in the chair, he is suddenly holding the teacup and saucer in his right hand, with his left hand in his pocket. (00:13:55)
Continuity mistake: For most of the scene where the Doctor and Donna find the dying Ood Delta-50, the front of his body is covered in snow. After he mentions "the circle", when the Doctor is asking what that is, the snow on Delta-50's chest is gone. In addition, the Doctor's hands change position between shots. (00:06:25)
Continuity mistake: When the Doctor starts scanning Korwin, he's holding the sonic screwdriver with his thumb on the end without the light. When Ashton says that Korwin sabotaged the ship, the Doctor's grip on the screwdriver has changed to one without his thumb on the end. Shortly after, he has his thumb on the end again. (00:06:10)
Factual error: The upper peninsula of Michigan is missing on the map of the US shown on Van Statten's monitor.
Suggested correction: In that era of Doctor Who the Earth was being invaded by aliens on a regular basis. Since the story is set in the 'future' year of 2012, anything may have happened to wipe that part of Michigan off the map.
I'm from Michigan and when I noticed the UP missing, I jokingly said to a friend "looks like Canada stole that finally"
Other mistake: After the Doctor regenerates, there's a close-up of the Thirteenth Doctor's eye with the console room reflected in it. The reflection is the same orientation as a shot from behind her immediately prior, which should not be the case. Take note of the positions of the door and time rotor in both shots. (01:00:45)

Continuity mistake: During the scene in the barn where the three Doctors have their hands on the button, when Eleven notices Clara crying and asks her what's wrong, the Doctors' hands go from stacked on top of each other on the button to only touching it with their fingers. (01:02:40)

Continuity mistake: After Clara enters the Doctors' cell, and they're talking about how the door was unlocked, shots inside the cell show the door about halfway closed, while a shot from outside looking into the cell shows the door wide open. (00:45:25)

Continuity mistake: Near the end, when the Doctor picks up the remote control and reads the words written on it, the position of his hands as he's holding the device changes between the close-up and the wide shot. (00:43:40)
