Continuity mistake: When Max is drunk on Liz's balcony, he draws a heart with his and Liz's initials in it. Then he lights it with his hand. Later, while Maria's singing, they show a flashback of him lighting it with the opposite hand.
Suggested correction: It wasn't a flashback, he went back there and re-lit it, using his other hand.
Continuity mistake: Twice during Maria and her mother's conversation about Valenti Maria turns away, but the next shot shows her looking at her mother again. (00:16:05)
Continuity mistake: Early in Amy and Valenti's conversation in his office, Amy's arms change continually from at her sides to crossed in front of her to gesturing. (00:28:30)
Continuity mistake: Near the end, when Michael is making his omelette, he switches from looking up to looking down between the shots on his right and left sides. (00:37:20)
Continuity mistake: At the end, when Ms. Topolsky is kidnapped by the special unit in the woods, the agent who grabs her gets out of the car on one side but when he gets back in with her he gets in on the opposite side.

Continuity mistake: When Michael, Alex, and Maria examine the camera in the kitchen, Maria turns to her right, the camera view shows her facing forward and swaying to her left, then cut back to her face and she's still facing to her right. (00:31:35)
Factual error: When Nasedo kidnaps Liz, he stops in a gas station to leave the FBI another clue. As he is pulling into the station, the sign above the gas pumps misspells the word "brakes" as "breaks."
Visible crew/equipment: When Sheriff Valenti first walks into the house of mirrors, you can briefly see the reflection a crew member wearing a white shirt.
Continuity mistake: When Isabel is in hysterics after she enters Max's mind to help locate him, Michael is holding her head in his hands. His hands abruptly vanish when she finishes talking about Pierce. (00:13:10)
Continuity mistake: As Michael, Isabel, and Tess are observing the hand scanner, the door and the agent using it move exactly the same way twice: once after he opens the door and again on a closeup on the scanner. Both times the agent steps to the left and the door begins to open. (00:16:25)
Continuity mistake: In the morgue, Michael flies back and hits the door twice. (00:23:20)
Audio problem: After Nasedo reveals himself in the morgue, there is a shot on Michael's face. There is no sound, but Nasedo can be seen in the foreground either talking animatedly or having an extremely localized seizure. (00:23:55)
Continuity mistake: After Max charges Pierce against the wall of the White Room, his hands change from gripping Pierce's jacket to merely set against his chest. (00:28:40)
Continuity mistake: In the scene where Pierce uses the VR goggles on Max, Max lowers his head while Pierce is talking to him, but the next shot over Max's shoulder shows that he hasn't moved at all. (00:30:15)
Continuity mistake: At the end of the interrogation where Max agrees to reveal the second communicator, Pierce skips from standing a few feet in front of Max and facing him to standing directly next to him with his nose at Max's cheek. (00:31:35)
Continuity mistake: Pierce reaches for the bone saw with his left hand but picks it up with his right hand. (00:37:00)
Continuity mistake: When Michael is trying to copy the fingerprints of the dead agent, Nasedo uses his power to transform Michael's outfit, which includes a jean jacket, into a suit and tie. When Nasedo hands him the comb, in the shot facing Michael he is in the suit and tie, but in the shot from behind Michael you still see him in the jean jacket. They show these camera angles twice.

Continuity mistake: The agent chasing Liz and Max at the bridge in the beginning abruptly switches from the left lane to the right. (00:03:00)
