Green With Evil (1): Out of Control - S1-E17
Continuity mistake: Up until Green No More in Series 2 (where the Green Ranger powers are destroyed), the Green Ranger's overshield constantly changes between solid-looking metal to flimsy-looking cotton. This is the most obvious difference between the Japanese and US footage.
Continuity mistake: When the kids leave the command center after being teleported there, in one scene they are all wearing different clothes and Trini's actress has changed as well - she has short hair. (00:11:22)
Continuity mistake: When we see the long shot of Rita blowing up the ground by the rangers, the clothes of the people in the long shot don't match what the rangers are wearing. Their positions are also different. (00:12:20)
Continuity mistake: The clips on the side of the Rangers helmets constantly disappear and reappear.
Continuity mistake: When in Tank mode and Megazord mode, the order in which the Rangers sit is: Billy, Trini, Jason, Kim, Zach. When switching between Tank mode and Megazord mode, Trini and Kim are switched - they are then back to their original seats once the Megazord has been formed.
Continuity mistake: When the Megazord is in Tank mode, the Mastidon's head section is attached to the center of the Zord. But when the Zord begins to stand up and form the Megazord, the Mastidon head section is gone.
Green With Evil (3): The Rescue - S1-E19
Continuity mistake: Rita mocks the Rangers from the cliff. Between shots, Rita's scepter changes, with the circular emblem changing from thin and circular; to thick and oval.
Continuity mistake: When Tommy gives Jason his Power Coin, Jason receives the Overshield and Dragondagger. The positions of Jason's arms change between frames when he receives them.
Continuity mistake: When the Rangers first morph, they engage in a battle with Goldar. When in this battle, Trini gets a black burn mark on her suit, which promptly vanishes. (00:09:50)
Suggested correction: This was a common occurrence during the entire run of the show, and it was theorized that the suits had an ability to self-repair to keep the Rangers from more harm. The only Ranger to never sustain damage to their suit was Tommy, since he had the Dragon Shield to protect him.
Yes, it's a common occurrence throughout the show, but normally it shows damage until the end of the battle/morph sequence. In the mistake listed above, the damage disappears between camera cuts in the middle of a fight sequence.
In the final battle between the Red and Evil Green Rangers, Jason shows a black impact mark on his chest, but it's gone in the next shot; maybe the suits do self-repair.
Continuity mistake: At the start we see Billy taking a karate class taught by Jason. In this shot you can the Rangers are wearing their communicators despite Billy having not created them yet. (00:04:30)
Continuity mistake: In this episode there are a few shots of Goldar with wings. However it is explained in season 2 by Lord Zed that Rita denies Goldar his wings, as Zed gives Goldar his wings back.
Continuity mistake: Bulk throws some papers in the air in his audition. However a lot more papers fall than were thrown. (00:05:00)
Continuity mistake: As Kim starts talking to Tommy she has 2 folders in her arms. One pink, one black. When she goes to show Tommy something from her folder, the black one disappears. (00:04:30)
Continuity mistake: When the Red Ranger is being held by the giant monster, his right arm goes from trapped to free and back between shots. (00:16:20)
Continuity mistake: During the battle involving the Zords, the Samurai pulls four pins out from his head, two each side, leaving a visible gap. However, this gap is filled very soon after, as the pins have reappeared. (00:16:00)

Continuity mistake: When Tommy throws the trash can lid at the Putty, it's upside down, but when the shot changes to the lid approaching the camera, it's flipped right side up.
Continuity mistake: When Kimberley and Trini are fighting the Putties and see the giant Minotaur pass by there is, what looks like, a small child between them. This child is not seen before or after this one shot. (00:11:09)
Continuity mistake: At the start of the episode, Bulk attempts to climb the rope and breaks it, pulling it down with several pieces of the ceiling coming down on top of him. At the end of the episode, this is suddenly all fixed with Trini climbing the same rope. The same gum and dirty spots are all around the top again with the ceiling appearing completely undamaged from before despite Bulk yanking it free and breaking the ceiling. (00:19:25)
