Factual error: Titan is depicted as a Moon-like landscape with no atmosphere. In actual fact, Titan has a nitrogen/argon atmosphere denser than Earth's, and a reddish-orange sky. This was detected almost thirty years before "The Invisible Enemy" was made, so somebody slipped up in doing their research.

Other mistake: When Marius injects the cloned Doctor and Leela into the real Doctor, the syringe is full of air, although the later graphic shows them in liquid.

Plot hole: Palmerdale and the others hear someone on the stairs. Skinsale opens the door and, despite Reuben being right in front of him, looks downstairs and calls out to the Doctor before turning and suddenly spotting the lumbering keeper.

Revealing mistake: The knife that Leela kills Meeker with is very obviously loosely attached to his clothes. It wobbles back and forth after he falls.

Continuity mistake: When Palmerdale wakes Harker up and asks him if he can use the telegraph, Harker answers him, then asks him to repeat the question, then answers again.

Continuity mistake: Lowe's "frozen" makeup changes between when he emerges from the freezer and when Leela brings him a cup of coffee.

Plot hole: Leela's decision to stay with Andred is very sudden. They barely speak during the serial.

Revealing mistake: The CSO caves are awful throughout, but one of the worst incidents is in episode three when Leela talks to Idas about a revolution and they both run right through a rock.

Revealing mistake: When K-9 shoots one of the infected men, the blast beam appears to come out of his eyes, then moves down to his snout as the camera moves.

Continuity mistake: The Doctor has his scarf when he first meets with the Vardans. When he returns to the TARDIS, it is on the coat rack.

Plot hole: The Nucleus of the Swarm is referred to on numerous occasions as being a virus. A virus is the simplest form of life yet recognised and is made up of bundles of DNA wrapped in a coating of proteins. They reproduce by attaching themselves to other lifeforms and releasing their genes into an unsuspecting host cell. Viruses do NOT lay eggs. Nor do they resemble prawns.(It would be more accurate to describe the Nucleus of the Swarm as a protozoan. These are simple animal lifeforms radically more advanced than viruses, around a fifth of which are parasitic although not all cause disease).

Continuity mistake: The TARDIS is missing its light the first time it's seen.

Continuity mistake: When the Doctor races out of the R1C with the fission bombs in episode four, Leela follows, carrying a shield gun. She steps into the cave and is only holding her knife. Later, she has her shield gun back.

Continuity mistake: The Gatherer gleefully states that being on the roof is a fine of 500 talmars. Later, Cordo says it is 5 talmars.

Continuity mistake: The countdown clock in Marius' lab speeds up and slows down as needed for the plot.

Underworld - S15-E5

Factual error: While it's true the centre of a planet would have no net gravity, the weightless condition would be everywhere, not concentrated in a convenient shaft.

Revealing mistake: The pacifier devise is obviously a stage lamp.

Revealing mistake: In episode two, when Leela and Mandrell are arguing about raiding the Correction Center, the camera bumps against something.

Continuity mistake: In the opening TARDIS scene, after the TARDIS lurches from the time scanner, the Doctor's hat rack can be seen standing upright. Then Leela picks it up in the next scene.

Horror of Fang Rock - S15-E1

Continuity mistake: In episode 4, Skinsale is murdered by the Rutan on the stairs right outside the crew room but later in the episode when the Doctor and Leela runs down the same stairs, Skinsale's body is not there.
