Deja Vu All Over Again - S1-E22
Continuity mistake: At the end of the episode, Phoebe's hairstyle changes from tied back when talking to Piper in the kitchen, to down and re-done in the time from the kitchen to the front porch.
Audio problem: When Evil Prue flings Piper and Phoebe back, you see Piper hit a pillar, but with Phoebe's voice of pain. When we see Phoebe fall behind a structure, we hear Piper's voice of pain.
Audio problem: When Piper, Paige and Phoebe run up to the attic to run from Shax, Piper says that they're about to find out if Paige is their sister, her mouth is shut when she is supposedly saying the sentence.
Other mistake: In the kitchen scene, Phoebe adds a mix to a pie crust but puts an empty crust in the oven. (00:04:10)
The Bare Witch Project - S7-E2
Other mistake: At the end of the episode when Phoebe is on the horse making her lady Godiva speech her jeans show at her waist. When they pan out she is "naked." When she starts to ride away her skin tone underwear shows. They cut away to Paige and Piper and when they come back to Phoebe riding away the underwear is covered by the long blond hair. (00:41:05)
Other mistake: When The Angel of Death comes to Leo in the car Leo asks him "can I at least see my son again?" which is an odd way to ask that question given that he has two sons.
Hulkus Pocus - S8-E9
Other mistake: When Piper is explaining to Phoebe that they don't have enough time to save Billie, she lists all the things they need to do ending with "fix us and then fix Billie..." This doesn't make any sense, since when she's saying it there's nothing wrong with them to fix. It's only after she says this that they come up with the plan to infect themselves. (00:29:54)
It's A Bad, Bad, Bad, Bad World (1) - S6-E22
Other mistake: Phoebe says there's nowhere to park except blocking the neighbor's driveway. We can see in the opening shot that there is no-one parked in front of the manor, and we see spots across the street. There is also enough room to park where she is without blocking the driveway. At the end of the episode, despite not moving it and also saying that she hasn't, it's not blocking the driveway any more. The patrol guy still shoots her. It's not illegal to park there unless it's blocking the driveway.
Plot hole: It is stated Leo is born on May 6th, 1924. Yet he was in his 20's as Piper's ex-lover in his past life around 1924. It also stated that that is when he died and became a whitelighter but the war wasn't until 1942. He died shortly after that meaning he didn't become a whitelighter until after 1942.
Plot hole: In the final episode Piper cast the spell with her mother Patty and grandmother Penny to get the Hollow out of (past) Piper, Phoebe, Paige, Billy and Christy. It worked. But in episode 4x13 Charmed and Dangerous, the only way to contain the Hollow is by good and evil working together... which wasn't the case in the final episode.
Other mistake: Prue tells Andy about her powers and he asks her if when she has kids they would inherit them - she says "If they are girls, yes" but later on in the series Piper has two boys who are both very powerful. (00:30:30)
Other mistake: When Prue astrals back to recite the spell to reverse the awakening spell, she and Phoebe don't actually complete the spell, they just recite the first paragraph "What was awakened for its sleep, must once again slumber deep" the rest according to the book of shadows is "return the disease to whence it came, so life can ease back to the same".
Size Matters - S4-E5
Other mistake: Since when are managers, on a trial basis no less, allowed to renovate and change the name of a club?
Suggested correction: Piper says in the episode that she allowed him creative control because the club wasn't doing well, and he was well-known to be good at what he does. It's made clear throughout the episode, by way of him telling her multiple times that if she doesn't like it that he'll get rid of it, that she still has full veto power on any changes.
Other mistake: Yenlo explains that he escaped limbo through the water in the clouds. And the yen master says there is no other water around. So why do the girls come in through a cave? If they have to leave through the water in the clouds they should enter through the clouds.
From Fear to Eternity - S1-E13
Other mistake: No one, including the cops, seem to think that all the victims' hair turned white is strange, as if getting scared to death actually turned your hair white.
Carpe Demon - S7-E14
Revealing mistake: When Miss Donovan is chained up, it is very obvious that her hands can easily slip through the shackles.
Continuity mistake: The first three episodes have zero continuity when it comes to Piper's love life. In Ep1 "Witch Trial" Piper is hooking up with the guy who is looking at the loan for P3. Then in Ep2 "Morality Bites" she is with Leo (presumably they have secretly been together for months as they discuss their many issues being able to talk when they're both so busy with magic crises and how Leo hasn't figured out how they can be together without clipping his wings). But by Ep3 "The Painted World" Piper and Dan are already together. They has been no discussion of Piper and Leo breaking up and zero mention of Piper and Dan getting together (in fact, the last time we saw Dan in Ep1, he and Phoebe were flirting pretty heavily). I am so confused and I do not at all remember these inconsistencies from when I first watched these episodes years ago. Were these shot and aired out of sequence or something?
Suggested correction: Your last statement is correct, which also invalidates your error. Ep3, "The Painted World" was produced before Ep2, "Morality Bites" but "Morality Bites" was aired first, out of sequence.
Suggested correction: Piper and Dan weren't together in The Painted World, they weren't together until she's a man, baby, a man. When Piper sees his dating tape.
Factual error: Melinda Warren says that after Matthew turned her in to the council, she was burned at the stake for being a witch, except witches were not burned at the stake, they were hanged.
