Sword and the City - S6-E8

Continuity mistake: When Phoebe is walking the Dwarf to the door after finding out Piper pulled the sword from the stone, he steps outside and goes into fast-forward, then stops and goes back to normal speed as he sniffs a flower. During this, in the background, you can see Phoebe, who freezes the whole time. (00:18:40)

The Bare Witch Project - S7-E2

Continuity mistake: At the end of the episode it is not Alyssa riding the horse to the café. Alyssa has a tattoo on her right ankle, which when we see when camera pans up the right leg the person riding the horse has no tattoo on the right foot, but when Alyssa rides away you can see the tattoo she has going round her right ankle.

Spin City - S6-E18

Continuity mistake: When the spider demon cocoons Piper, Paige throws a blue potion (the vanquishing potion) at her, Chris catches it and jumps over the couch, knocks Paige to the floor and Phoebe kicks Chris. The potion falls out of Chris hand and rolls into Paige's open hand, it has now turned yellow (the antidote) Paige grabs it as she stands and throws it at the spider demon again and it has now turned back into the blue potion.

Happily Ever After - S5-E3

Other mistake: If all the objects in the fortress are originals from the fairy tales, why are the ruby slippers red? In the original story the slippers were silver, they were made red for the MGM movie, so they would be the brightest thing on the screen.

Witchness Protection - S7-E10

Continuity mistake: Midway through the episode, the Seer is looking through the sisters' photo album. Several of the photos actually include everyone who was present for the events depicted, leaving nobody to take the photo. And since the events were shown in various episodes, we know that no photo was taken at those times by any automated method.

Bride and Gloom - S3-E13

Revealing mistake: When Prue talks about her relationship with Justin near the beginning of the episode, she says that she finds him to be very predictable, lacking in mystery or "savoir-faire." That doesn't make sense, though, as "savoir-faire" means "know-how." She probably meant "je ne sais quoi," that is, "I know not what."

Love's A Witch - S6-E5

Continuity mistake: When Chris orbs to the manor after Paige has returned home, his hair is brushed to lay mostly flat and behind his head, with a part on the left side of his scalp. When he comes into the kitchen shortly thereafter, his hair is shaggy and unkempt, with a part in the middle.

The Day The Magic Died - S5-E15

Continuity mistake: In the beginning, Paige is wearing a shirt with the scrunching sleeve on the right and slanting on the left. In following scenes, the scrunching sleeve is on the left and slanting on the right. (00:07:40)

Power Outage - S3-E7

Continuity mistake: During the training drill, Piper fumbles for a knife on a table in front of her. In the shot of the table, she loses hold of the knife as she pulls her hand up, but when we cut to a long shot, it is back in her hand.

Shay

Witches in Tights - S5-E5

Continuity mistake: Just after 'the Aggressor' tears the picture of the girls, they all change back to their normal clothes, however in one shot, Paige's arm is visible and she is still wearing her costume. (00:34:35)

My Three Witches - S6-E6

Plot hole: Phoebe's empathic powers result in Gith accidentally creating a world based on Jason's desires instead of Phoebe's. But if Gith was just using his own powers to perceive the Charmed Ones' desires, why did Chris give him a list of those desires?

Answer: That was a mistake in the series.

Also, the episode is called 'Babies First Demon' and this alone tells us that it's Wyatt's first time in the field (so to speak) and as the sisters say, they've done a good job of clearing up their own magical messes so far. Nobody really knows how advanced Wyatt's powers are, or will become. That's why the cleaners appear, to nip things in the bud. The Cleaners didn't want to chance that the sisters could clean Wyatt's magic up, just incase they couldn't. Therefore intervening themselves, just in case.

Chosen answer: When the cleaners first surface in season 6 it is mentioned that usually the girls have been able to clean up their own messes (as seen in season 3 episode 22 when Phoebe turns back time thus concluding in the death of Prue).

Answer: Because they wanted them to fix their own mistakes.
