Continuity mistake: When Mal and Nandi are talking and drinking on the couch, the position of Mal's left arm keeps changing between shots. In both the closeup shots of Mal and Nandi, his arm is resting next to her on the couch, but in the shot of them both, his arm is behind her.

Continuity mistake: In the flashback where Mal and Zoë meet Jayne for the first time, every close-up of Mal and the one close-up of Zoë shows their hair blowing in the wind. But in the shots of them together, their hair is completely still.

Continuity mistake: At the beginning of the episode when Mal, Jayne and Zoë are ransacking the destroyed ship for supplies, after Mal says "Vault's sealed" there is a shot of Mal as he inspects the door hatch before asking Jayne for the explosive gel gun. The ring around the door hatch's handle from the explosive gel gun is already visible in this shot.

Visible crew/equipment: Tracey threatens the crew with a gun on the bridge when he hears that they're about to call the cops. He says that he'll blow a hole right through Wash if they do. There is a shot of Mal saying "You mailed your ugly business to Zoë and me, Tracey." As he says this, a crew member's hand can be seen above them (through Serenity's windshield on the left of the screen) grabbing one of the pieces of fallen ice.

Continuity mistake: When Buffy and Riley are in Buffy's room, Buffy holds Riley's hand with both of her hands and says to him, "Is there anything I can do?" But in the following shot she is holding Riley's hand with only one of her hands.

Revealing mistake: Look at the pilot in the cockpit of the fake ambulance (that the crew made to infiltrate St. Mary's hospital) as it lands at the main entrance. It looks nothing like Wash, not even lifelike.

Revealing mistake: Towards the end when Oz says to Veruca, "You don't wanna find out what I am", and partially turns into a Werewolf. You can tell that both of the actors were filmed separately for the CGI's sake as Oz stands out from the picture more than Veruca does. The same effect is obvious during "Doppelgangland" in season 3 where there are 2 Willows onscreen at once.

Shindig - S1-E6

Continuity mistake: At the beginning of the episode, Mal, Jayne and the slavers are playing holographic snooker. One of the slavers gets ready to hit the white ball and leans over the table with his snooker cue. The holographic balls then start to malfunction. In the shot of them malfunctioning the snooker cue that the slaver was holding has disappeared from the table only to return in the following shot.

Shindig - S1-E6

Visible crew/equipment: Right after Kaylee tells Mal that she's not speaking to him there is a wideshot of the ballroom dancers. In the archway at the back of the room Joss Whedon, the director/creator, walks into shot wearing a Hawaiian shirt, shorts and a baseball cap with headphones around his neck. Just as he raises his hands to speak to the dancers the shot cuts to Inara and Atherton.

Continuity mistake: When Mal is fighting Crow on the ship, he falls over the containers of medical supplies, knocking some of them over. A few shots later when Jayne shoots Crow, the containers are standing upright again.

Continuity mistake: When the crew are in the cockpit talking about Miranda, Jayne is in the background holding his head with his left hand. 5-6 shots later he is holding it with his right hand. On the cast commentary of the DVD, Adam Baldwin says that he did this on purpose "just to mess with continuity". (01:03:35)

Revealing mistake: When River goes crazy on Serenity, knocks out Jayne and Simon and then holds Mal at gunpoint, you can tell that the gun she is holding is quite heavy because her arm wobbles. But when she cocks the gun it is completely still. In the cast commentary of the DVD, Joss Whedon reveals that the gun was in fact held up by a C-stand for that particular shot because the gun was too heavy for Summer Glau to hold.
