Continuity mistake: Hank boards Boris' helicopter, but just before he does the helicopter is shown in a wide shot with no one nearby. Hank boards and a guard closes the door from the exterior. Seconds later the helicopter takes off and a wide shot shows no one on the ground. The guard appears out of nowhere and then vanishes.

Flight - S8-E24

Continuity mistake: "Flight" (8-24): At the five minute mark, Meredith, wearing scrubs, frantically leaves Christina to go search for her missing husband, Derek. Yet when we see her again a few scenes later she is wearing a jacket.

Factual error: In Season 9/episode 21, Alan is staying at Lindsey's house when he goes to the basement to solve an electrical problem. He is electrocuted by a fuse box but fuse boxes were replaced by circuit breakers beginning in the 1950s. Lindsey's house as seen in a previous exterior shot is of modern vintage.

Continuity mistake: When the sub is being strafed by the Japanese fighter, John Wayne is wounded as he enters the doorway with his back toward the fighter. Yet, when he is taken to his quarters, it is his chest that is wounded and his back is untouched.

Continuity mistake: Jennifer Beals' blouse mysteriously changes from white to blue while they are in the CIA secret bunker prior to heading out for the final confrontation.

Later that day during an autopsy, she is wearing a red blouse and then at the diner with her new beau, she is back to the black blouse.

Witness - S1-E7

Continuity mistake: While Reese and the witness are hiding in an apartment, the door is opened by the student to dismiss the bad guys with the chain lock on the door. The door is open about 18 inches and yet when the student closes the door the chain lock is normally sized about 4 inches and is still attached.

Continuity mistake: The two girls are in a Goodwill store shopping and Caroline, the blonde, is carrying a pair of shoes throughout a long scene and then she turns to leave the conversation empty handed. (00:06:00 - 00:08:10)

Continuity mistake: When the girl working in the hospital sees a man from her flash forward, she runs after him carrying several hospital client files. As she runs through the hospital stopping people, the files magically disappear from her arms.
