Audio problem: In the scene where Kananga's henchman, Adam, is being interrogated by Sheriff Pepper by his car at the riverside, the boat chase arrives and two boats jump clearly overhead, disillusioning Sheriff Pepper and allowing Adam to make a quick getaway. However, as Adam is getting away in his car, screeches can be heard coming from the tyres, but they are on a dry dirt road. (01:29:40)
Factual error: Clifton James' character is "Sheriff J. W. Pepper of the Louisiana State Police". A sheriff is not part of any state police organization - a sheriff is an elected COUNTY official. (Interestingly, in Louisiana, they are not called counties, they are called "parishes"). (01:30:20)
Continuity mistake: While Bond is in the boat chase, after his boat engine gets shot, he beaches it into a grassy area. The men in the boat chasing him slide up on the bank all the way into a swimming pool and Bond gets out of his boat to go steal a different one. As the henchmen's boat comes to a stop in the pool, you can see them start to stand up and point their guns towards Bond. Then the shot changes to Bond driving off in his new boat and past them, and they are again starting to stand up to point guns. (01:31:20)

Continuity mistake: During the chase scene when several men in boats are chasing Bond in a boat, one of the bad guys steals a black speedboat from the Sheriff's brother-in-law. The bottom of the black speedboat collects white scuff marks as the chase continues, but the scuffs increase and decrease erratically, indicating that the sequences were not shot in the order we see in the film. (01:35:15 - 01:38:25)

Visible crew/equipment: Four patrol cars are chasing Bond and the villain by the road. Bond and the villain arrive at a dead end and they jump over the road with their boats. The four vehicles collide just after. Look carefully and you will see that the right-seat passenger of the red car and the driver of the blue one wear a yellow protective helmet. (01:36:45)
Other mistake: In the boat chase, when Bond comes to the shipyard, the goon can't find him. But there would be obvious foam/wake marks and waves after Bond's boat for several minutes. (01:37:20)
Revealing mistake: When Bond and Quarrel goes to shore there are clear shadows and sun reflections, showing it was actually shot at daytime. (01:40:55 - 01:41:40)

Revealing mistake: There are two different shots of two red explosives being placed on the ground next to a bamboo stalk, with the explosives being in a different position in each shot. However, you can see that it is the exact same location filmed for both explosives: the same stalk, the same leaves, and the same dirt pattern. They just stuck a bomb in the same spot and filmed it twice to make it look like two different bombs placed. (01:43:45)

Continuity mistake: When approaching Solitaire, the voodoo priest's grip on the snake changes between shots. (01:44:15)

Continuity mistake: When the guy with the snake is shot, Baron Samedi reappears and you can distinctly see the bloke holding the snake again. (01:47:00)

Factual error: At the climax, Baron Samedi falls into a coffin crawling with snakes, which promptly bite him to death. But a closer look shows they're mostly harmless species—boas, pythons, racers, and green tree snakes. None of these are venomous; boas and pythons kill by constriction, and the others are basically harmless. The scene tries to sell a terrifying venomous death, but the reality is he just got dropped into a reptile house full of non-killers. (01:47:34)
Audio problem: Before Bond and Solitaire get into Kanaga's den at the end of the movie, we see two guards running. However, there are only footsteps from one person. (01:48:45)
Revealing mistake: When Bond and Solitaire are tied to the orange thing you can see a wire on the bullet which makes it fly across the room. (01:52:45)
Other mistake: When Bond is tied up with Solitaire and turns on his watch - only the gas bullet comes flying. But that could not be the only loose metal object in the room. Kananga's blade for example. And even if he held on to it he would feel the magnet pulling it. (01:52:50)
Continuity mistake: When Bond and Solitaire are on the lift where they were put by Mr. Big, look carefully at Bond's watch. The face is concave with the crystal over it. When he uses the cutting function, the whole face is now rounded like a dome. (01:52:50 - 01:53:55)

Continuity mistake: Before Bond and Solitaire are submerged with the sharks, the stuff on the operator's table changes positions between shots. (01:54:10)

Visible crew/equipment: When Kananga and Bond are fighting a cable protrudes under Kananga's jacket, it even lifts up his clothes. Seemingly the mic cable. (01:54:27)

Continuity mistake: When Bond and Kanaga falls into the shark pool, they fall apart from each other. However, when we see them from under water, they are holding onto each other. (01:54:35)
Deliberate mistake: In the scene where Bond and Kananga are fighting underwater, Bond shoves the bullet into Kananga's mouth. Then, Kananga rises up and explodes. He explodes like a balloon, with not much blood at all. (01:55:00)
Plot hole: After Bond defeats both Whisper and Kananga, all the other red-shirt henchmen that were in the room with them seem to have disappeared. When Bond came down into that room with Solitaire, and several times during the scene down there, Kanenga's men were seen all over the room in various locations. Yet after Bond beats the two main baddies, the other guys have all gone and are not attacking Bond. (01:55:15)

Chosen answer: One assumes that the card that 'foretold' that Rosie was a double-agent was either sent by Felix Lighter of the FBI/CIA or by Solitaire herself. But, more than likely, it was Bond's faithful sidekick Quarrel, Jr. (the fisherman/boat captain on San Monique).
