Continuity mistake: After Indy exits through the sewer with his hair all wet, he escapes Kazim's men. When he turns around a block his hair is nicely combed and loose, instead of sticky. Not too plausible when you're running for your life and your head is soaked in petrol.
Continuity mistake: The distance between Indy's boat and the big red and white ship keeps increasing and decreasing randomly.
Other mistake: In the motorbike chase scene there is a lot of flying road dust even in front of the bike in the lead. Probably from the camera vehicle.
Visible crew/equipment: When Indy Sr. is shot, two big stage lights are reflected on his glasses.
Continuity mistake: In the zeppelin, after Indy says "no ticket", everyone searches for their tickets and waves it at Indy. When the angle changes, a frame later, the people are searching for their tickets, no one is showing one.
Revealing mistake: In the temple, when Indy walks on air, he briefly becomes semi-invisible due to the matte effect.
Continuity mistake: The crater Indy's car falls into becomes bigger and way deeper between the first and second shot.
Continuity mistake: When the driver of the tank is shot and he changes the direction of where the tank is going, Indiana gets up, with his strap still stuck on the nozzle. However, in the next shot, he's become clear of being stuck. There's no way he could have gotten his strap undone so quickly.
Continuity mistake: Near the end, in the "Leap of Faith" scene, Jones steps off the edge of the abyss and discovers an ingeniously-camouflaged stone bridge. The camera pans to show the camouflage effect from another angle, and it's obvious that the stone bridge is only about one foot thick from one end to the other. Relieved, Jones scampers across to the other side, and the camera cuts again to show that the bridge is now a full three feet thick.
Continuity mistake: Before the propeller chops the boat, Elsa leaves and looks at Indy. Right then, the camera is reflected on her boat's windshield.
Continuity mistake: After tracing the cross, the drawing goes in the diary. Despite being in the water afterwards, when we next see the drawing, it is dry, as is the diary itself.
Revealing mistake: On top of the tank, several German soldiers are lined up, then all are shot by Indiana Jones with one bullet. The blood squibs can be seen underneath the soldiers' uniforms.
Continuity mistake: When the flocks of birds smash into the fighter plane after Henry Jones Senior frightens them into the air, the plane's propeller stops spinning, but the next shot of the plane coming down to crash shows the propeller still spinning.
Other mistake: At the library in Venice, Brody and Jones point out the Roman numerals in the library, wondering what they mean. Elsa comes over, and they show her. In front of her, behind the rail, is an obvious plate of glass. You can see her reflecting in it. The plate disappears in the next shot and never returns. The plate is not there in the longer shots. Where did it come from?
Suggested correction: I just watched the scene about five times and could not find any glass or reflections. All I saw was their shadows against the wall. Furthermore, it would make no sense for them to set up a big glass plate for one shot and only one shot.
I re-wound it several times. It's right at the beginning of the shot when Elsa comes from the right. It's brief but there. This was on a TV version. You may be watching a fixed version on DVD or Blu-ray. I'm sure the mistake has been corrected in later releases.
I think you are mistaking a shadow for a reflection. Or possibly it's just a video artifact. As the previous correction stated, why would there be a sheet of glass there?
I'm standing by my correction in this case. It just patently makes no sense why they'd set up a big pane of glass on their set for one shot then remove it. It wouldn't serve any purpose other than to catch light, and it would have also lightly messed with the way the background looked and the shadows the actors are casting. If it was a TV version, there's no way to tell what master they're using, how old it is, how it was stored, etc. It could have video artifacting or damage that caused the "reflection." (Probably just a light "ghosting" effect.) Heck, sometimes even your TV settings can cause issues like that. If you use any sort of motion smoothing or frame interpolation, those can create weird double images sometimes because the software doesn't always work properly.
Continuity mistake: When Indy is at the Lion's Head reading from the diary, he has the diary in his right hand and moves his right hand away from him towards the rock wall. The next shot is a long shot and shows Indy holding the diary with both hands in front of him by his midsection.
Continuity mistake: When Indy first shows up in the uniform inside the blimp, he turns to squarely face a man sitting in a chair near the wall. When it cuts to the next shot of the German, he's turned ninety degrees to the right. Too fast between shots.
Continuity mistake: In the library before he goes through the hole in the floor, Indiana takes the map out of the diary and gives the diary to Marcus. When Indiana gets caught trying to rescue his dad, they take the diary from him and say Marcus has the map.
Suggested correction: You've forgotten the scene at Donovan's apartment. There, Indy could have cut the page and give it to Brody off-screen.
Continuity mistake: When the soldier walks up the steps to attempt the first trial with his sword drawn, in the shots from the front, he's holding the sword on his right side. In the shots from behind, he's holding it on his left side.
Continuity mistake: During the boat chase, after the one boat blows up, another comes up behind Jones and Elsa. Jones starts turning the boat. In the next shot, the man in the boat behind them is shooting at them. They're not turning, they're going straight.
Continuity mistake: Before they make the second fighter pilot crash, Henry hands his bag to Indy. In that shot, the handle of Henry's umbrella is closer to Indy's left shoulder. But, in the next shot from in front of Indy, the handle is closer to his right shoulder. Then, when Henry pulls the umbrella out, the handle is back near Indy's left shoulder.
