Visible crew/equipment: During the pursuit scene in San Francisco, there's a long shot where the fire truck passes in front of the camera just before the two police cars get separated. During that shot, you can see a camera crew on the top of the truck, in front of the ladder. (01:31:40)
Suggested correction: It's not a crew member, it's James Bond.
Continuity mistake: When Bond grabs Stacey after the Zorin Airship blows up, you can see the fireaxe resting between them. The camera shot changes to one above them and the fireaxe has now gone. (02:01:00)
Suggested correction: The axe isn't gone. What's happened is the handle is hiding behind Bond's foot. The red head of the axe simply blends into the paintwork of the bridge and given the distance of the shot it's hard to spot.
Continuity mistake: When Zorin's men attacks Stacey's house, she throws a jar at one of them. A huge part stays on his head and he doesn't remove it when he escapes with the other. When Stacey is taking the broken parts on the floor, you can see that same huge part. At the place it is on the floor, the man had to put it gently on the floor, something he didn't. (01:13:40)
Suggested correction: Actually, you do see this just as the thugs are escaping you see him take it off his head.
Continuity mistake: When Bond and Stacy leave the mine shack, him in a company jacket and her in overalls, you can see she is wearing red heels which Bond comments "it is the union" to a worker. Later when May Day rips the overalls off Stacy she is wearing white shoes to match her dress. (01:37:20)
Suggested correction: Bond never says that to a worker. What he says is "It's Women's Lib. They're taking over the teamsters." When he does she still has white/silver shoes on.
Continuity mistake: When the Rolls sinks its back end is pointing upwards; the front end going down in a steep angle. But when Bond is getting out of it and gets air from the tires it is upside down. And the surface is not far away. It is impossible that it could have turned over. (00:51:10)
Revealing mistake: When May Day winches James Bond to get the bomb out of the mine shaft, you can see the cogs of the winch don't move when she turns the handle. (01:51:30)
Continuity mistake: When Bond is in Mr Howe's office at San Francisco City Hall, the exit-only door is marked "Divisions of Oil and Mines" when he leaves. But when Stacey is fired, the door is marked "Division of Oil and Mines." (01:07:00)
Suggested correction: It's two different doors that are seen. Both go to the same office but it's two different doors.
Visible crew/equipment: When the thugs' limousine departs after the attack on Stacey's house, a boom mic is reflected on the side windows. (01:13:50)

Continuity mistake: The locomotive that Scarpine drives in the quarry throughout the mine sequence has an all-green radiator, but when he and Zorin come out of the mine, the top of the radiator is black. (01:48:30)
Suggested correction: That's because they are on a different locomotive. If you watch closely, the one they ride in parks by the hut that ends up under water. They take one that is on a separate track and raised rock.
Audio problem: Scarpine's comment about Bond hanging from the mooring line doesn't match his lip movement. (01:55:30)
Suggested correction: If you watch closely you will see that it does match up.
Audio problem: When Bond is knocked unconscious into the back of the Rolls Royce, everyone gathered around moves away. Listen carefully and you can distinctly hear a technician/assistant say, 'Mind that door.' It cannot be dialogue from the film due to the way it is said. (00:50:05)
Suggested correction: It is definitely not "Mind that door." The henchman who closes the door says "Au château" ("[Back] To the castle") to his two female colleagues, which makes perfect sense if they are still in France.
Continuity mistake: When Bond leaps from the bridge in his pursuit of May Day, he lands on and crashes through the boat's roof. When we see May Day jumping onto the speedboat, the boat's roof is now completely intact. (00:20:00)
