Visible crew/equipment: During the scene where Neo is talking to Agent Smith in the park area where Neo was talking to the Oracle, there is a close-up on Agent Smith's face. In his sunglasses you can see a bright white screen to reflect the light onto the faces of the actors. This isn't visible in any non-reflected angles. You can also see the cameraman on the other side. (00:51:15)
Visible crew/equipment: At the Black Gate, Sauron's forces march towards Aragorn and his army, there are close-ups of Pippin drawing his sword and of Merry taking a deep breath. The next shot is of Orcs running to the right of the screen. As that shot opens, click five-six frames in and two crew members are seen between the Orcs on the right of the screen, click a few more times and more crew members, one with a hat, are visible in the middle, between the Orcs. Visible crew/equipment: When Bruce goes back to the building toward the end of the movie looking for God and finds it deserted, as he runs through it, you can see the shadow of a figure (a crew member) on one of the columns on the left. It even slightly moves as he's approaching the ladder as if trying to duck away from behind. (01:13:55)
Visible crew/equipment: In the scene after the huge fight with the Buddists, Jack Nicholson speeds off in his Land Rover and cuts into traffic to get away from the monastary. If you look three cars back there is a state trooper weaving through both lanes to make sure the set is closed. He stays visible throughout the scene. (01:07:35)
Visible crew/equipment: Aboard the Jolly Roger after Smee jumps overboard, Peter flies up to catch his sword and a pirate who yells, "I got him!" grabs hold of Peter's ankle. When Peter flies off in the wide shot, as the pirate holds his leg, he passes the mast and not only are the four wires attached to Peter's backside pleasingly visible while he flies, the two wires at the pirate's shoulders are visible as well. Visible crew/equipment: In the scene where Martin Lawrence and Steve Zahn are in the red station wagon that Steve Zahn was unable to commandeer and was slapped silly for, they drive by a glass building and you see the camera crew and equipment on a platform in front of the vehicle, moving with them.
