Continuity mistake: When the bomb under the bus is being defused, it's counting down and beeping once every second. We see the timer at 0:45, then cut to the face of the woman defusing it - counting the beeps when we cut back it should be on 0:45, but it's jumped up to 1:06. Camera pans down to her face, then back to the bomb, and it's now on 0:43, but going by the beeps it should be on 0:36. We cut away again for 5 more beeps, cut back and the bomb's only just ticking onto 0:41.
Testimony of Evil (Dead Men Don't Laugh) - S1-E6
Continuity mistake: At the start, the glass on the left is poisoned. The guy picks up the glass on the right, but still ends up dying.
Continuity mistake: When Mary talks to Bobby D at the crime scene, her hair moves back and forth over her left shoulder between shots throughout their conversation.
Continuity mistake: When the detectives are in pursuit in the alley, they are followed by a police car. In all shots from the front it has its blue lights on, while they are off in all shots from the rear.
Continuity mistake: When Irina meets XIII on the river dock, she leaves her car, closes the driver side door and walks to XIII. Moments later, when XIII is shot, she runs back to her car, where the door is wide open.
Continuity mistake: Kate and Donny are having a drink after they close the bar. She's alternately holding the bottle by its neck or much lower down. (00:24:00)
Who Killed the King of the Country Club? - S2-E11
Continuity mistake: When Burke questions him on the golf course, Conrad Hill is completely in shadow for all the frontal shots, but in full sunlight in the over-the-shoulder reverse angles. (00:14:00)
Continuity mistake: After he's dragged behind the horse, MacKenzie's facial abrasions keep changing completely from one scene to the next. And one day later, they've miraculously healed enough to be covered by one little bandage.
Continuity mistake: Lund tells Gregorio that he got a text from a friend in DC, who ran the names of Yates and Blakely through a government database and found something. When it cuts to a young man walking, in the interior shot from inside the police cruiser the rearview mirror is missing from the windshield, but in the exterior shots the rearview mirror is properly attached. Continuity mistake: Neils Jesper is handcuffed in the back of a prison transport van with his seatbelt on, once the van violently comes to a stop and the van doors open he's shot not wearing a seatbelt. (00:30:45 - 00:31:30)

Continuity mistake: Joanna Lumley has long hair tied back while driving her car but much shorter hair subsequently.
Episode #2.3 - S2-E3
Continuity mistake: Jack and Georgina pull into parking spot next to another vehicle. In next shot, as they walk away from their Mercedes, there is no vehicle in the next spot with no time for it to have pulled out. (00:28:15)
Continuity mistake: In the scene in the courtroom towards the end where Alicia is talking to the security guard who is in the witness stand, he glances over at the jurors. When they cut back to the jurors a few seconds later, they are wearing different coloured shirts. (00:33:30)
Continuity mistake: The team is in their plane with 3 digital clocks on the wall. The clock on the right jumps from 4:x:x to 22:x:x in the next scene which happens within minutes, and then back to 21:x:x.
Split Second to an Epitaph: Part 2 - S2-E3
Continuity mistake: When originally interviewed, Ironside holds up the police sketch. It is evident that it still is a bit rough as it has a line across the eyebrows. Later in the televised broadcast, the picture is completed, no roughness at all.
A Community of Victims - S3-E3
Continuity mistake: Officer Ripley's shoes change from black to white as he chases the ambulance, then back to black.
Episode #3.5 - S3-E6
Continuity mistake: Sidney is attending a pre-wedding dance in the camp he is visiting wearing a light colored shirt. When he leaves the dance floor to chat with other attendees, he is now wearing a green sweater over his shirt.
Continuity mistake: Overcome by mysterious toxic fumes, Bad Cooper loses control of his car and rolls it on the roadside. The vehicle tumbles but lands upright; and, as it comes to rest, the interior camera shot looking out shows a relatively minor crack in the far left side of the windshield. Camera cuts to an exterior frontal shot looking in at Bad Cooper, and now the windshield is shattered all the way past center, with the left side partially caved-in. Camera cuts back to an interior shot looking out, and the windshield is, again, largely undamaged. Camera cuts back to exterior and the windshield is, again, thoroughly shattered. Another interior shot looking out, again, shows minor damage.
