Visible crew/equipment: During Bud and Coffey's fight in the lab, we see a lightbulb swinging from side to side between the two men. If you look closely, you can see a hand (belonging to a third person) pushing the lightbulb. The only people in the room are Bud and Coffey. (01:44:16)

Visible crew/equipment: About half way through there is a shot that shows Jonathan sitting in his living room. On the right-hand side of the screen we can see the windows and it's dark outside. Even so, you can see a cameraman holding a camera moving along the window filming Jon from behind.

Suggested correction: I just watched this scene. It's shot in broad daylight. There are no big "film" lights on either side of the pier, rather regular light posts you'd normally find on a pier. On the DVD timestamp listed, it's a shot of Robert Duvall looking for D-Fens. The shot cuts forward to a tracking shot of him running. I don't see film lights at all. Shots like this wouldn't need film lights. Maybe diffusers and reflectors but not huge film lights.
Visible crew/equipment: In the scene where Laura is being attacked in the desert, the camera swings wildly. During this swing, the entire crew becomes visable for a split second. You can see Clint Eastwood (who also starred in the film) sitting in his director's chair, wearing a white t-shirt and watching the filming closely. (01:19:54)

Visible crew/equipment: In the director's cut, there's a scene where the bar-owner is tied up to the bed. A few seconds before it cuts away, a crew member's leg can be seen on the left hand side.
Visible crew/equipment: When they are moving the catapult, if you look above Robin and Azeem, you can see a crew member in the window above them. He is wearing all black, but you can tell he is trying to get out of the shot.
Visible crew/equipment: When Donnie and his Dad are in the car, talking, prior to nearly hitting Grandma Death, in the window behind Donnie's Dad is the reflection of a vehicle which says "Darko" clearly on it. There's also the reflection of a clapperboard on the backseat. (00:21:00)
