Revealing mistake: After Little Red Riding Hood has been summoned to the field, there is a shot with Little Red Riding Hood in the foreground of the shot, while the other characters talk on the balcony in the background of the shot. During the shot, Rebecca is leaning her head over the edge of the balcony and starts talking. The other characters turn their heads to face her, but their heads appear to be facing the area behind where she is standing and not looking in her direction. (00:09:32)

Revealing mistake: When Joey asks Tea, "You gotta use the little girl's room?", after she stops walking, Bakura's mouth is missing from his face. (00:02:26)
Revealing mistake: When Joey asks Tristan, "Embarrassed 'cause you nearly destroyed the world as we know it?", the Millennium Puzzle is drawn incorrectly; the eye on the front side of the puzzle is way too high up and not in the centre where it is usually shown. (00:02:59)
Village of Vengeance: Part 4 - S5-E28
Revealing mistake: When Yugi realises that they are in the room where they store all the food for the palace, there is a shot from above where Joey shouts, "Please tell me this isn't happening." In this shot, Yugi, Tea and Tristan are drawn without faces. (00:12:40)
One Step Ahead: Part One - S5-E10
Revealing mistake: A live feed of the duel between Kaiba and Zigfried is displayed on screens in the arena that are positioned behind them. Whenever the screens are shown, another version of the same screen can be seen displayed on the feed. However, the same live feed cannot be seen. Instead, a white light is displayed on the screen and not another version of the live feed.
A Brawl in a Small Town: Part One - S5-E8
Revealing mistake: When the audience members are cheering, in the shot where the announcer is heard saying "The KC Grand Championship is heating up", there is a man wearing a red jumper, visible in the top right corner of the frame, whose mouth is missing from his face. (00:01:45)
Child's Play - S5-E5
Revealing mistake: When Vivian is shuffling Rebecca's deck, the illustrations on the cards are missing. (00:04:09)
Revealing mistake: When Yugi gets off the boat and sees everyone standing on the dock, Solomon can be seen to the left of the shot with his right arm randomly raised in the air. There is no reason for him to be doing this and he remains completely still while doing so. There is a painted-over freeze frame a few seconds later which shows both of his arms in the air and cheering, which may explain why his right arm was raised in the earlier shot even though he wasn't cheering at that point. Unwanted Guest: Part One - S5-E1
Revealing mistake: After Kaiba has finished explaining the KaibaCorp Grand Championship, there is a wide shot of the office where Kaiba says to Mokuba, "Good idea." In the wide shot, Kaiba's body appears to be unnaturally small, and his arms and legs appear even tinier. (00:07:36)
The Dark One Cometh: Part 2 - S5-E32
Revealing mistake: When Bakura activates the spell card Cursed Twin Dolls, two dolls appear on the field. The doll wearing a pink dress while holding a black box is missing its left arm in the shot of the dolls appearing on the field. The dolls have two arms every other time they are shown, so this is an animation mistake. (00:12:23)
The Dark One Cometh: Part 2 - S5-E32
Revealing mistake: When Yugi activates Lengard's special ability, in the shot of him saying, "Whenever one of my monsters is damaged, the result is automatically transferred to Lengard, so my life points are safe," the cards he is holding in his hand are missing the illustrations. (00:11:04)
Revealing mistake: When Seto begins twirling his Millennium Rod in order to attack the monster, the rod is shown spinning around in circles, but his hand and fingers don't move. They stay completely still while the rod twirls in circles. (00:07:10)
Sinister Secrets: Part 3 - S5-E14
Revealing mistake: When Leon says "I can't even control my own monsters," the cards in his hand are missing the illustrations. (00:07:37)
Sinister Secrets: Part 3 - S5-E14
Revealing mistake: When Kaiba walks into the control room, he says, "Alright, initiate Protocol KC-514." When he says this, there is a woman with blue hair in front of him sitting at a computer, whose left hand is shown typing, but her hand is missing the keypad. (00:04:10)
Sinister Secrets: Part 2 - S5-E13
Revealing mistake: When Zigfried is saying, "There is only one true victor here, and that is Schroeder Corp," there is a brief montage of different groups of people watching the duel. During the montage, there is a shot of a family in the living room. A boy can be seen sitting on the floor with duel cards, and the cards that are face-up are missing the illustrations. (00:16:55)
Sinister Secrets: Part 1 - S5-E12
Revealing mistake: Just after Leon summons his Iron Knight, there is a shot of him saying, "I win." In the shot, there are cards visible in his hand. Then, midway through the shot, the cards abruptly disappear from his hand when he moves it. Then, the cards reappear in his hand at the end of the shot. (00:20:28)
One Step Ahead: Part One - S5-E10
Revealing mistake: When Kaiba and Zigfried finish shuffling each other's decks, in the shots of them handing their decks back to one another, the bottom cards on their decks are missing the illustrations. (00:06:30)
A Brawl in a Small Town: Part One - S5-E8
Revealing mistake: When Kaiba and Mokuba are in the control tower, a security guard walks into the room. His reflection can be seen in the window and begins walking forwards at a rotated angle. Then his body enters the frame, but he is shown walking in a straight line, which shouldn't have been possible given the angle he was walking at in the reflection. (00:02:07)
Unwanted Guest: Part Two - S5-E2
Revealing mistake: When Yugi and Rick walk down the steps of the duelling platform after the duel has finished, Yugi is holding onto Rick's deck before handing it back to him. In this shot, the front side of one of Rick's cards can be seen, and it is missing the illustrations. (00:12:20)
Unwanted Guest: Part One - S5-E1
Revealing mistake: When Mokuba is showing everyone the new duelling field, in the shot of Joey saying "Not too shabby," the Millennium Puzzle that Yugi is wearing is animated completely wrong. Instead of the hole in the centre and the designs that are shown on the front side of the puzzle, there is only one awkwardly drawn shape. (00:12:06)
