Obelisk the Tormentor - S2-E5

Continuity mistake: When Kaiba is testing the new duel disk system, Mokuba is watching from the room above. In the background, there is a wall that is a light teal colour to the left of the shot and another wall further back that is a dark teal colour. When the shot cuts to Mokuba saying "You're crazy if you go through with this, Seto," the wall behind him is entirely the dark teal colour. (00:05:40)

Continuity mistake: Just after Kaiba has won the duel, a scientist says, "Shut down the power source right away," whilst standing next to Mokuba. It then cuts to a closer shot of Mokuba, and the scientist standing next to him cannot be seen. (00:15:21)

Duel Identity, Part II - S1-E30

Character mistake: Yugi mistakenly says if he doesn't get a monster out to defend himself Mai can attack him directly, which wouldn't be possible since the rules of Duel Monsters during the Duelist Kingdom arc forbade direct attacks on defenseless opponents. That rule wouldn't be implemented until the Battle City arc.
