Continuity mistake: In S1 E15 "Enigma", about 30 minutes in, Gibbs takes out his flask and the engraving is "For Good Luck, Your Loving" with the bullet on it. At the end of this episode it's engraved with "Jethro, For Good Luck Love Shannon and Kelly".

Frame Up - S3-E9

Continuity mistake: On their way to their crime scene, Ducky and Ziva accidentally fall into a river. Just a few moments later, when Ducky is observing the severed leg, his clothes are relatively, if not completely, dry.

Continuity mistake: When Tony sees Ziva arrive at work, he is seated at his desk and starts to lean back in his chair. He is then instantly seated upright with his arms on his desk, then leaning back in his chair again. (00:01:35)

Continuity mistake: When McGee shows up late in Abby's lab, and Tony ribs him about what he's wearing, Tony holds up McGee's writing pipe with one hand. In the next shot, he's holding the pipe with two hands. (00:13:05)

Hiatus: Part 2 - S3-E24

Continuity mistake: The year Gibbs joined NCIS is inconsistent. It is revealed in this episode that he joined the NIS in 1992, the same year it became NCIS. But in the JAG crossover episode "Ice Queen", Gibbs tells Commander Rabb that he has been an agent for 19 years, which suggests that Gibbs joined NIS in 1984. And in "Singled Out" in season 4, McGee mentions that Gibbs has been an agent for roughly 16 Years, which would put Gibbs' joining NIS in 1990.

Continuity mistake: When Ziva, Ducky, and Dr. Burns find the graveyard, Dr. Burns is visible between Ducky and Ziva's shoulders. She then disappears in the following wide shot, and appears again in the closeup. (00:39:40)

Frame Up - S3-E9

Continuity mistake: In the scene where Chip first brings the evidence to Abby (after getting it from Mr. Palmer), in one shot, her spiderweb tattoo is on the right side of her neck, and Chip is standing to her right. In the next shot, her tattoo is on the left side of her neck, as it normally is, and Chip is standing to her left. (00:08:20)

Kill Ari: Part 2 - S3-E2

Factual error: After Gerald gets back to NCIS HQ, he notes that he wasn't able to drive Ducky's Morgan; a stick. Abby brags about knowing how to drive a stick since she was ten. When asked what she drove that had stick, she answered, "Red '47 Ford half-ton with four on the floor and Bubba riding shotgun." When asked who Bubba was, she answered that he was the best coon dog in Jefferson Parish. If in fact Abby grew up in the city of New Orleans, as stated by McGee in the episode before, the dog would not have been the best coon dog in Jefferson Parish but Orleans Parish. No part of New Orleans is inside the Parish of Jefferson. If it was truly the best coon dog in Jefferson Parish then Abby would have lived in Metarie or Kenner, Louisiana, both of which are inside the parish limits of Jefferson Parish.

Silver War - S3-E4

Continuity mistake: When inside the Smithsonian, Tony is fooling around with a bear. The baseball hats on all 3 agents have black trim on the brim, then white trim, then black trim. Zeva says her hat has a 9mm hole, and pokes a finger through a hat with a black trim. Gibbs says it is for ventilation. Then she drives home, the hats have white trim, and hers has no hole.

Head Case - S3-E15

Visible crew/equipment: Just near the end, as Ducky faces the other M.E. in Autopsy with his guilt, the camera shows a person reflected in the silver wall (who would be standing beside Ducky). It's not Ducky, as the M.E. is looking at and talking to Ducky with a different angle. The person appears to be dressed in medical scrubs, but the M.E. and Ducky are the only two in the room. (00:40:45)
