Factual error: Joker, Penguin and no less than four henchmen all manage to hide in the Batmobile's trunk and leap out when it enters the Batcave. Even for Batman, this is stretching credibility more than a bit. There's no way the Batmobile's trunk could hold six adults and still close (and it was closed). Visible crew/equipment: The Batmobile, responding to Batman's remote control activator, is supposedly driving itself through Gotham City's streets. But in one shot, even though he's crouching down, you can see the head of the film crew member actually driving the car. (00:03:00)

Continuity mistake: After Alfred pulls out the key, two pieces of the little blue time-shifting box have apparently fallen to the floor. Joker snatches one of them up while struggling with the butler. But when Batman and Robin leap in for the next shot, the second piece has vanished from its spot on the floor. (00:20:30)

Continuity mistake: After starting the sand in the giant hourglass, Clock King puts on his alarm clock top hat. Between distance and close shots, the clock reads a few minutes to three, then Three o'clock in the closeups.

Visible crew/equipment: After Batman gets out of the Batmobile, you can see the lighting rigs, boom mic, camera and crew reflected on the Batmobile's side.

Answer: There were three well-known actors who played Mr. Freeze, each one a different height, weight, and physique. As these were notable guest stars, the costume designers would adapt the costume to best fit each actor who likely would want to avoid comparisons to the other Mr. Freezes, incorporate their preferences, as well as refresh the look, rework whatever didn't work in previous versions, and so on.
