The Catwoman Goeth (2) (a.k.a. A Stitch in Time) - S2-E34
Visible crew/equipment: When Alfred and Batman take off from the Batcave on the Alf-cycle (a nod to Batman's quick head-dip), it cuts to the exterior scene at the Run Down Factory. When the GCPD patrol car pulls up, in their closeup the movement of a crew member's reflection is visible on the triangle vent mirror, at the bottom left side of the screen. (00:10:30)
The Bat's Kow Tow (2) - S2-E30
Visible crew/equipment: While Allan Stevens interviews Batman and Robin they have surprise visitors, and as Catwoman and her Kittens take their places pointing the weapons at the audience, the black tape mark is visible on the floor at Catwoman's feet. (00:06:45)
It's the Way You Play the Game (2) - S2-E26
Visible crew/equipment: After Batman and Robin have "set another youth on the road to a brighter tomorrow," they walk away from Andy, then when they stand beside the Batmobile, the vertical reflection of the moving boom pole/mic is visible right beside Robin's reflection, on the left side of the windshield. (00:03:50)
Revealing mistake: After Shame and his baddies steal Bruce's limo. Bruce shows Dick and Alfred that he had the remote control unit keep the Bat-cycle (with Alf-cycle) following them at a safe distance. In the shots facing the Bat-cycle, attached to the front wheel there's a rope pulling it. (00:17:05)
The Clock King's Crazy Crimes (1) - S2-E11
Visible crew/equipment: After the Dynamic Duo climb up the side of the building they enter through the window, and a chalk T-mark is visible on the floor where Batman stops when he sees Clock King sitting at a desk. Then, as Batman and Robin slide around on the floor coated with watch oil, when Clock King stands up we can also see the curved chalk mark on the floor by his feet. (00:21:50)
The Clock King's Crazy Crimes (1) - S2-E11
Continuity mistake: When Clock King steals his favorite painting from the gallery, as he places it through the wall hole note the strips of tape adhered to the paintings back, and its hanging wire near the bottom (it's upside down). In the next shot from the other side of the wall hole, the painting is now turned the opposite way (it's right side up), note the tape strips. (00:16:25)
Visible crew/equipment: While Batman and Robin are tied to the spit and rotating, when Batman says, "If we could just shake this turn spit pole loose," in the next shot note the shadow of a crew member moving their arm up, at the lower right corner of the screen. (00:03:35)
Walk the Straight and Narrow (2) - S2-E2
Visible crew/equipment: At stately Wayne Manor, when Bruce and Dick are practicing their archery, two white tape marks are visible on the grass; one for Dick and the other for Alfred. (00:23:05)
Visible crew/equipment: When Minstrel and his henchmen race down the stairs, the sound pole rigging shadow is moving at the very top of the screen. (00:04:00)
The Clock King's Crazy Crimes (1) - S2-E11
Continuity mistake: During the fight in the art gallery, the papers on the desk to the left of the screen are scattered. But in some shots of the fight progressing, the same papers have straightened themselves back into nice, neat stacks again. (00:17:00)
The Clock King Gets Crowned (2) - S2-E12
Factual error: Trapped in the overturned hourglass, Batman and Robin lie flat and begin furiously digging in the sand, making the hourglass roll out the door. But to cause such rapid movement, they'd need to be applying their weight against one of the glass sides (like a hamster in a ball). Just digging in the sand wouldn't get them anywhere, at least not that quickly. (00:04:15)
Shoot a Crooked Arrow (1) - S2-E1
Continuity mistake: The arrow Archer fires into the window frame in Commissioner Gordon's office is, depending on the camera shot, angled up, then down, up, down and up again. (00:07:30)
Continuity mistake: After Annie drops the chandelier on them, Batman and Robin are knocked unconscious. No one's had time to move either of them, but when Andy comes into the saloon in the next shot, Robin has somehow shifted positions on the floor: he's not in the same spot or position he originally fell in. (00:21:30)
Hot Off the Griddle (1) - S2-E3
Revealing mistake: During the fight in the museum, one of Catwoman's henchmen slams into the wall beside a window. The set wall buckles and caves in, leaving the bottom edge of the window bent and jutting out. A few shots later, though, both wall and window have repaired themselves. (00:14:00)
Scat, Darn Catwoman (2) - S2-E41
Audio problem: At the end, Catwoman falls from the roof into the water below. But we never hear a splash. (00:21:15)
The Zodiac Crimes (1) - S2-E37
Continuity mistake: Robin is holding an upright 8-foot pole when Venus sprinkles her knock-out glitter on the Dynamic Duo. When the shot cuts and they fall to the floor, the pole has completely disappeared. (00:22:10)
Continuity mistake: Between takes at the end of the final fight scene, most of the chairs shift positions around the table, the wooden partition screen that fell over during the scuffle has somehow folded itself, and a white cloth that was draped over one of the chairs has disappeared. (00:22:00)
The Clock King Gets Crowned (2) - S2-E12
Continuity mistake: Batman tosses a large white box at one of the henchmen. The box lands on the round dais of the clockworks, but as that dais begins to rotate, the box disappears. It pops back up a few shots later, now sitting on the stationary corner of the clockwork platform. But as the fight rages on, it vanishes again between shots. (00:21:30 - 00:22:10)
The Minstrel's Shakedown (1) - S2-E5
Continuity mistake: When Alfred brings the lunch tray down to the Batcave, Batman is holding a soldering iron, working on the electronic bug. When the shot cuts to a close-up, the soldering iron vanishes, and he's holding a clear plastic probe instead. (00:10:15)
Continuity mistake: While Batman is telling Octavia that she can go free, his hands are either straight down at his sides or clasped together at chest level, depending on the shot. (00:06:15)

Answer: It's wasn't unusual for multiple actors to play one character on this show. The villains on the TV Batman were played by guest stars, not regular cast members. George Sanders, Otto Preminger, and Eli Wallach played the part at various times. They may simply have only wanted to play the part once or twice, or they were later tied up with other projects, making it necessary to cast someone else. Other characters, like Catwoman, were also played by more than one actress.
