Visible crew/equipment: As Mr. Jay is talking to Bruce and Dick, on the wall behind Dick you can see the shadow of the boom mic. (00:23:45)
Revealing mistake: As the Batmobile exits the Batcave, if you watch the exit closely, you see the shadow of the back end of the car come to a stop, meaning that it was at the wall of the set. (00:05:10)

Visible crew/equipment: As O'Hara goes to move his patrol car, the boom mic and lighting equipment is reflected on the windows.
Visible crew/equipment: When the photographer is getting a picture of the princess, you can see the shadow of the boom mic on him.
The Joker's Hard Times (2) - S2-E38
Continuity mistake: When the Joker directs Gordon and O'Hara to look at the statue of Justice, they look at it from a front view. But when Joker comes out of the Police building where he left them frozen, it's from behind the statue, rather than in front.
The Pharaoh's in a Rut (2) - S1-E28
Revealing mistake: When Bruce Wayne is strapped to the stretcher hurtling toward the cliff, after he breaks through the "300 Ft. Drop" sign, we cut to a different angle where the wire pulling the stretcher is visible. (00:02:40)
Factual error: When Bruce, Dick, and Aunt Harriet are looking at the globe, Bruce corrects Dick's mistake about Lima being the capitol of Ecuador and says "No, as you can see, I was right. It's the capitol of Peru." and points to the globe to show him. Based on the position of Europe facing the camera, he's actually pointing to somewhere in northern Canada. (00:02:35)

Other mistake: Not only can Caesar Romero's moustache can be seen, but the line between the white makeup and his skin on his neck. Also, when Robin's face came out of the clay, his mask was almost around his mouth. A moment later, the mask is back in place. Later, his face, mask, and body is spotless.
Flop Goes the Joker (2) - S2-E58
Revealing mistake: After Bruce stops the governor of the mobile, it can be seen that the weights are painted Styrofoam balls.
Enter Batgirl, Exit Penguin - S3-E1
Revealing mistake: The panel that Batgirl makes her entrance through can be plainly seen as a prop mounted on top of the set of stairs she comes down.
Continuity mistake: At the end, when Minstrel is cornered by the Dynamic Duo, he pulls out his sparking remote unit. After being told it won't work, as O'Hara cuffs him, the unit that was in his hand disappears.
Continuity mistake: Batman's utility belt is normal in the beginning, but suddenly has a triple sized compartment when Batman goes to get up on the catwalk with his rope. (00:03:30)
Surf's Up! Joker's Under! - S3-E10
Revealing mistake: When the Dynamic Duo are locked in the board blanks, you see the jets where the smoke and flame come out.
Answer: It's wasn't unusual for multiple actors to play one character on this show. The villains on the TV Batman were played by guest stars, not regular cast members. George Sanders, Otto Preminger, and Eli Wallach played the part at various times. They may simply have only wanted to play the part once or twice, or they were later tied up with other projects, making it necessary to cast someone else. Other characters, like Catwoman, were also played by more than one actress.
