Continuity mistake: Jack and Humphrey are following the taxi in Jack's grey car, the registration number is DG60 SGY when they start following the taxi and LN63 FEJ when they park in front of it. (00:14:50 - 00:16:25)

Continuity mistake: When the DJ is handed the LP at the start the way it's held changes position between takes. (00:02:35)

In the Footsteps of a Killer - S6-E7
Plot hole: Spoiler. The killer needs to get rid of two women. One is his mistress that he just dumped and is acting psychotic about it, the other is his wife. He kills the wife and makes it look like it was his mistress to murder her. So she goes to jail, where she will die two years later due to pneumonia...and in all this time, she never once says anything about the affair! He has been extraordinarily lucky, because had she said anything, and she had absolutely no reason not to in her circumstances, the case would have not been so open-and-close, they would have considered the idea that he could have been an accomplice, but even assuming the past detective (which DI Richard Poole called a good detective) was a total fool, at the very least Jack and his team would have found a trace of this controversial alleged affair in the files and solved the case much earlier. But no, the plan was to send his mistress to jail and that somehow made her cease to be a threat to him, when instead she'd have been much more dangerous to him.
Episode #1.3 - S1-E3
Continuity mistake: The principal admits his lie and brings the team to his real destination. At the entrance, Commissioner Selwyn Patterson's is standing with his hands behind his back, in front of him, and behind his back again at the cut. (00:23:20)

Continuity mistake: When they find out that the chocolates are poisoned, Camile puts the label down. Camera cuts and she does it again. (00:08:40)
Episode #1.2 - S1-E2
Other mistake: Camille sits down and orders a large cocktail with a list of what she wants in it. This is then brought to her within 20 seconds. Not possible given the distance from the bar and the order. (00:04:00)
Switcharoo - S9-E5
Continuity mistake: Emma brings Neville a plate with a sandwich, cut in half, while he is in the laundry room. Neville picks up the half of the sandwich closest to him, looks at it, and puts it back on the plate. He then picks up the half of the sandwich farthest from him, holds it for a brief minute, then puts it back on the plate. Only, the half farthest from him is already on the plate when he puts the one he's holding back. You need to look closely as this one was tough to catch. (00:33:20 - 00:33:45)
Switcharoo - S9-E5
Continuity mistake: Emma brings Neville a sandwich in the laundry room. Neville picks up the half closest to him and puts it back down. He then picks up the half farthest from him, holds it for a brief minute. When he puts it back on the plate, the half farthest from him is already there and he puts down the half closest to him. This was a tough one and I had to watch a few times. (00:33:20 - 00:33:45)
Revealing mistake: When they break the door to the nurses room down because it's locked, you can see the lock is retracted so there was no need to break the door down. (00:05:10)
Revealing mistake: The doctor closes the blinds on the ward and picks his phone up and makes a call. However, you can see the screen never comes on so how could he choose a contact to ring?
Continuity mistake: When JP is talking to Marlon who is having lunch, Marlon's napkin keeps alternating between blowing in the wind and hanging freely.
Episode #1.5 - S1-E5
Continuity mistake: After Leon Hamilton is stabbed, Richard Poole gets out the whiteboard and says "this time it's personal." Yet the whiteboard has some writing on it and some pictures. If you look at it closely, it is Benjamin Lightfoot, the guy killed in the next episode when Richard is sick. So the whiteboard has everything from the next episode before it happens.
Episode #4.6 - S4-E6
Continuity mistake: When JP announces that the SIM card was sold in Isla Jonas, Dwayne in the foreground is holding his hands on the buckle of his belt the opposite way compared to the wider angle. (00:36:25)
Answer: Most likely they had only tropical exotic drinks, he wanted an old fashioned English beer.
