Continuity mistake: The episode starts out in a flashback of three college kids hanging about a pool. One of them is a nerdy guy who passes off a key fob to the other dude who leaves. A murder happens, and then the scene morphs to the current Judge Crawford, but there is one major flaw. Crawford is about 60ish, and the key fob is from the '90s era. So, the timeline is off.

Fifi's First Christmas - S4-E12

Continuity mistake: Brian's hair is the standard bowl cut he's sported throughout the series to this moment, even on meeting new girl Sarah and their piano lessons. When Sully, Mike, and Robert E find him mounting a horse to go look for lost Sarah, it's suddenly gelled back for a more 'older' look.

Continuity mistake: Or perhaps just algorithm: When Indy and his friend are delving through the webbed cave in search of treasure, the friend turns a bit to reveal a pile of tarantulas suddenly covering his shoulders, descending his chest, the same as Indy, who suddenly turns, revealing his backside covered in them. When cut, prior, their backsides were empty. The spiders didn't just fall from the ceiling onto their backs; we would have seen this. Purely for creep factor and nothing more.

The Streets - S1-E2

Continuity mistake: The journalist/reporter lady's hair changes between shots when she is interviewing Hooker at the Bible killer's empty car. In a single close-up, her hair is 'blowing' like an isolated intimate moment between her and Hooker, while in the other shots her hair is in place.

Continuity mistake: When Lucy, disguised as the Agent, leaves Mr. Reilly's office for the last time, she suddenly reappears on his balcony overlooking the flowers as he gives dictation to his secretary. When she'd have had to venture out of a window from another part of the office elsewhere. Purely a sight gag for the show, no less.

Chapter Two: Vecna's Curse - S4-E2

Continuity mistake: When El advances on Angela and her friends at the skating rink and wallops Angela in the face with the skate, her aim is upwards at the lower cusp of Angela's nose for the impact. When Angela's friends sit her up in the following shot, the area between her eyes is cracked and bleeding instead as she screams in agony.

Continuity mistake: Tammy Thompson's voice changes octaves as she sings the National Anthem. At the start, she's in a normal mezzo range. When the camera cuts from her to the back-and-forth dialogue between Steve, Robin, and Vickie, her voice then changes downscale to a very noticeable alto.

Continuity mistake: When Michaela enters the homestead after handing Katie off to Matthew, it immediately cuts to her in the master bedroom upstairs, pouring water into the bowl to splash on her face, as she suffers another pregnancy attack when she wouldn't have been able to manage climbing the stairs in her delicate condition.

Continuity mistake: Mike has just pulled into the driveway and honked, as the family waits excitedly for him to come in and share his great news. Then, a second later, he's coming around the counter to greet everyone, instead of a car door or patio shutting.

Continuity mistake: Jake is ordered to remove the children (Colleen and Brian) from the courtroom, and escorts them outside. Yet as Dr. Mike resumes her protest to the judge, Jake is suddenly back in his frontal seat as Bailiff.

Continuity mistake: Elizabeth's hair length doesn't match while on The Ferris Wheel. The shot from the ground is very long hair 'draped' over the back of the seat. Yet, in the following close shot of her, it is from the chest up, so only the very bottom strands would be visible over the edge.
